    2. I am an applicant of Hazardous Waste Generator Registration pursuant to RA6969 and its IRR
    DAO 1992-29 AND Revised Procedures and Standards DAO 2013-22;
    3. Based on our records, as far as I know, there are no Air Pollution Source Equipment
    operated / installed in our facility. Thus, exempted for permitting requirements
    under R.A. 8749 or Philippine Clean Air Act of 1999.
    4. I am executing this “ Affidavit of No Air Pollution Source Installation” in compliance
    with the requirements of the DENR-EMB Region 4A for the Hazardous Waste Generator
    Registration.
