A RESOLUTION TO AUTHORIZE THE CLAIM OF CHEQUE AT THE SWAD OFFICE IN


TAGBILARAN CITY, BOHOL AS A FUND GRANT FOR THE ASSOCIATION.

WHEREAS, The Association (MATAUB SUSTAINABLE LIVELIHOOD ASSOCIATION) has


decided to authorize the President, Juan Dela Cruz of the Association to claim the cheque
at the SWAD Office In Tagbilaran City, Bohol

WHEREFORE, RESOLVED AS IT IS HEREBY RESOLVED, authorizing the President of the


Association, Juan Dela Cruz, viz:

RESOLVED FURTHER, to provide the Implementing PDO a copy of the resolution for its
information.
