Dear Participant,
The PUP-Ragay, Camarines Sur Branch, through the Bachelor of Science in Business
Administration major in Human Resource Management 3-1 students, taking Organization
Development subject this Summer 2023, under the supervision of Dr. Dorren D. Arenque, would
like to ask for your permission to please participate in this survey, being one of our respondents.
Your responses will be a great help and valuable in understanding the leadership qualities and
behaviors that make a positive impact. Rest assured that the information that will be gathered
will be treated with utmost confidentiality and shall abide by the provisions set forth under
Republic Act 10173, also known as the Data Privacy Act of 2012.
