1.0 PURPOSE

The purpose of this document is to detail the requirements pertaining to excavation works and
its associated activities for MIE project management, HSSE personnel, engineers,
superintendents, supervisors, sub-contractors and vendors. It also provides for the
communication and compliance with the relevant health, safety, security and environmental
information as supplied by the client to the project management team.

This Excavation Procedure has been prepared in accordance with the MIE ISO 45001 : 2018
Management System, which is certified and compliant with the said standard and
requirements and the plan will describe the HSSE management system of MIE that will be
implemented during the execution of CIVIL STRUCTURE (Package A) and PIPING,
MECHANICAL EQUIPMENT ERECTION, ELECTRICAL & INSTRUMENTATION,
INSULATION & PAINTING WORKS FOR PROCESS AREA (Package B) FOR PHASE 1 (50
KTPA) FOR PROPOSED 200 KTPA NITRILE BUTADIENE RUBBER (NBR) LATEX PLANT
ON LOTS 2604,2605,2615 AND 2616, MUKIM TANJUNG DUABELAS, DAERAH KUALA
LANGAT, SELANGOR DARUL EHSAN (F44SL NBR LATEX PLANT PROJECT) FOR TOP
GLOVE SYNTHETIC RUBBER SDN BHD.

This procedure applies to all work activities executed by the Package Contractors and their
subcontractors, as well as the Package Vendors under the management of MIE

2.0 SCOPE

This guideline covers all excavation and penetration activities performed and includes any
ground penetration or excavation 1500mm or greater from the finished ground level (including
the thickness of concrete or other coverings). Any penetration whereby a wall, roof, floor or
ceiling or other covered area is penetrated regardless of depth will be subject to the provisions
of this procedure.

These requirements are applicable to all excavation and penetration activities conducted by
Contractor and Subcontractors engaged on any portion of the works associated with the
Project.

3.0 ACRONYM AND DEFINITIONS

Barrier A physical structure which blocks or impedes something.


Benching Means terracing or stepping the sides of an excavation to prevent a
cave-in. Forming one or more horizontal levels or steps.
Batter The inclination of a slope expressed as vertical units/ horizontal units
Contractor Contractor engaged by OWNER to undertake the PACKAGE WORKS
for a PACKAGE.
Competent A person, who has acquired, through training provided by Contractor
Person HSE Trainer, qualification or experience, or a combination of these, the
knowledge and skill enabling that person to perform the required task
correctly.

Dewatering The removal of water from the water table by means of a mechanical
pumping unit in order to minimize water in the excavation / soil.
Engulfment The surrounding and effective capture of a person by a liquid or finely
divided (flowable) solid substance that can be aspirated to cause death
by filling or plugging the respiratory system or that can exert enough
force on the body to cause death by strangulation, constriction or
crushing.
Excavation Any man-made cut, cavity, trench, depression or hole in the earth, or a
face of earth formed after rock, sand, soil or other material is removed
(such as a trench, ditch, shaft, well, tunnel, pier hole, cutting or caisson
or a hole drilled in the earth).
Live Cable Any service that is energised (placed into service) or has the potential to
be energised and cause harm e.g. electrical cables, pressurised gas
pipes and data cables.
Protective A method of protecting employees from cave-ins, from material that
Systems could fall or roll from an excavation face or into an excavation, or from
the collapse of adjacent structures. Protective systems include support
systems, sloping and benching systems, shield systems, and other
systems that provide the necessary protection.
Trench A narrow excavation (in relation to its length) made below the surface of
the ground. In general, the depth is greater than the width, but the width
of a trench (measured at the bottom) is not greater than 15 feet (4.6 m).
If forms or other structures are installed or constructed in an excavation
so as to reduce the dimension measured from the forms or structure to
the side of the excavation to 15 feet (4.6 m) or less (measured at the
bottom of the excavation), the excavation is also considered to be a
trench.
Utilities Any piping, wiring, conduit or other equipment that serves as a conduit
for electricity, fuel, water, compressed air, sewer, and
telecommunication, etc., to or from a facility.
Note: The utility may be above or below ground.

Contractor and sub-contractor management is responsible for ensuring that workers are made

contractor management and Supervisory Personnel to identify potential hazards, assess the risk
of injury or harm to a person resulting from those hazards, and consider the means by which the
risk may be reduced to As Low as Reasonably Practicable (ALARP).

6.0 F44SL 13 LIFE SAVING RULES

The 13 Saving Rules applicable to this procedure is:

Rule 10: Obtain authorisation before excavation or entering a trench.

Excavation is work that involves a man-made cut, cavity, trench or depression in the earth’s
surface formed by earth removal.

A trench is a narrow excavation made below the surface of the ground.

To obtain authorisation, you need to get the excavation certificate approved. Buried services
mean underground services including pipelines, electrical/instrument/earthing cables, sewer
pipes and underground tanks.

7.0 GENERAL REQUIREMENTS

Prior to starting work, operations involving excavation, trenching, and shoring will be assessed
using the JHA and Risk Assessment for excavation. Both the JHA and Risk Assessment portions
of the registers are required for this and other operations to ensure that all hazards associated
with the work are identified, the likelihood and severity are understood, and control measures are
defined that will effectively reduce the risk.

 All Permits and associated Excavations shall be performed in accordance with the DOSH
requirements, Permit to Work and this excavation procedure
 DOSH Regulations pertaining to excavations, stipulates those excavations deeper than four
(4) meters shall be in accordance with the design and drawings of a Professional Engineer

7.1 MANAGEMENT OF RISKS

Risks associated with excavations include:


 person/s falling into an Excavation;
 person/s being trapped by the collapse of an Excavation;
 person/s working in an Excavation being struck by a Falling Object;
 person/s working in an Excavation being exposed to an Airborne
Contaminant;
 person/s drowning or engulfed
 person/s exposed to live services

NOTE: Where a service has been located and cannot be identified it shall be deemed
live; (Excavation near Live Services);
 All Excavations will be adequately compacted once backfilling is complete;
 Equipment powered by an Internal Combustion Engine shall be controlled so
that workers shall not be exposed to airborne contaminants;
 No work is to be performed within 2m of the edge of an Excavation without a
Physical Barrier (Hard Barricade);

 All Excavations deeper than 1.5m in depth, or left open for greater than one
(1) shift shall be barricaded with a Hard Barricade (physical protection) to
prevent personnel from falling in, and signage posted;
 Personnel entering Excavations shall be protected from engulfment by
Sloping and Benching, or an adequate Support and Protective System in
accordance with this Procedure;
 Compressed Gas Cylinders shall not be placed in an Excavation. Feed hoses
shall not be left in Excavations when not in use;
 Slope safety (including rock falls, benching face access, catch fencing, etc.);

7.2 CONFINED SPACE

Any excavation greater than 1.5m in depth shall be considered a Confined Space
until proven otherwise by a Competent Person who will assess the excavation based
on the criteria below:

 Is not intended as a regular workplace (i.e. continuous employee occupancy);


 Has restricted means of entry or exit;
 Is large enough and so configured that an employee can bodily enter and
perform. assigned work; and
 Is at atmospheric pressure during occupancy.

In addition, this space has at least one of these characteristics:

 It contains or has a potential to contain a hazardous atmosphere;


 It contains material (solid or fluid) that has potential for engulfing an entrant;
 It has an internal configuration such that an entrant could be trapped or
asphyxiated by inwardly converging walls or by a floor which slopes
downward and tapers to a smaller cross-section;

8.0 PRE - JOB PLANNING

The primary hazard of excavation is the risk of encountering hidden or Underground Services
and Utilities that may trigger serious consequences such as electrocution, leaks and asset
damage.

Another major hazard is injury and engulfment of personnel from a collapse. Personnel entering
excavations shall be protected from engulfment by using adequate sloping and benching, or an
adequate support and protective system.

Pre-planning Activities shall consider specific hazards involved in order to:

 Avoid injuries to workers, or interruptions to construction activities caused by disturbing


existing Underground, or Concealed Services such as power cables, gas lines, fire
mains, telephone cables, fibre optic cables, water mains, effluent lines etc.;
 Avoid damage to existing Underground or Concealed Services, and to above ground
equipment and buildings
 Avoid Trench / Slope collapse and personnel engulfment.

8.1 IDENTIFICATION OF BURIED UTILITIES/STRUCTURES

Before commencing work on any excavation or trench, utility companies or owners


will be contacted and advised of the proposed work, and to determine the location of
underground installations such as sewer, telephone, water, fuel, electric, gas lines,
etc.

The estimated location of utility installations, such as sewer, fibre optic, telephone,
fuel, electric, water lines, or any other underground installations that reasonably may
be expected to be encountered during excavation work, will be determined prior to
opening an excavation.

Owners will be contacted within established or customary local response times,


advised of the proposed work, and asked to establish the location of the utility
underground installations prior to the start of actual excavation.

When excavation operations approach the estimated location of underground


installations, the exact location of the installations will be determined by safe and
acceptable means, such as hand digging and pot holing.

8.2 COLLECT SITE DATA

Prior to commencing any Excavation and Penetration Activities the Contractor/ sub-
contractor shall gather available information about the site conditions and location of
Utilities. This shall be achieved by referring, to Site Plans, Engineering Drawings (As-
Built), Design Drawings and by holding discussions with stakeholders that are
knowledgeable about the site.

8.3 PRE-WORK SITE INSPECTION

Prior to an excavation
 Inspect the site to determine what HSE measures must be taken.
 Underground Services such as sewers, telephone, electric, water lines, or any
other Underground Utilities that may be encountered during Excavation Work
shall be physically located and marked.
 A Utility Locator may be used to accurately determine the proximity of Utilities
prior to Manual Excavation, to confirm the location of Buried Utilities.
 The location of identified Services shall be visibly marked on the surface in a
manner that will last until the Excavation is completed.
 Utilities left in place shall be protected by barricades, shoring, suspension, or
other means as necessary.
 Implement controls to minimise potential risks and hazards associated with
large Open Cut type Excavations based on ground conditions. the site data
collection may be required.

9.0 PROTECTION OF EMPLOYEES

Protection systems will have the capacity to resist all loads that are intended, or could
reasonably be expected, to be applied or transmitted to the system.

Sides, slopes, and faces of all excavations will be scaled, benched, rock-bolted, wire-meshed,
or secured by other approved equally effective means. Portable trench boxes or sliding trench
shields may be used instead of shoring or sloping. Such boxes or shields will be of strength at
least equivalent to the sheeting or shoring that would be required for the composition of the soil
or material in which the trench is being made

Protective systems for excavations greater than 6 meters in depth will be designed by a
Professional Engineer.

10.0 BENCHING AND BATTERING

Benching and Battering of Excavation Walls can minimize the risk of soil or rock slipping onto
the excavation. Benches should be wide enough to stabilize the Slopes and to prevent material
from the top falling down to the working area. They should also be sloped to reduce the
possibility of water scouring.

Safe Cut Slopes in rock shall be designed by an appropriately experienced Civil engineer. Safe
Cut Benching and Battering in rock fill can be constructed in accordance with the angles, in
accordance with this procedure.

11.0 SOIL CLASIFICATION

Type A soil is cohesive and has a high unconfined compressive


strength; 1.5 tons per square foot or greater. Examples of type A soil
include clay, silty clay, sandy clay, and clay loam. Soil cannot be
Type A
classified as type A if it is fissured, if it has been previously disturbed, if it
has water seeping through it, or if it is subject to vibration from sources
such as heavy traffic or pile drivers.
Type B soil is cohesive and has often been cracked or disturbed, with
pieces that don't stick together as well as Type A soil. Type B soil has
medium unconfined compressive strength; between 0.5 and 1.5 tons per
Type B
square foot. Examples of Type B soil include angular gravel, silt, silt
loam, and soils that are fissured or near sources of vibration, but could
otherwise be Type A.
Type C soil is the least stable type of soil. Type C includes granular soils
in which particles don't stick together and cohesive soils with a low
unconfined compressive strength; 0.5 tons per square foot or less.
Type C
Examples of Type C soil include gravel, and sand. Because it is not
stable, soil with water seeping through it is also automatically classified
as Type C soil, regardless of its other characteristics.

Benching

12.0 SLOPING / BENCHING SYSTEM

The slopes and configurations of sloping and benching systems for excavations 5 feet (1.5
meters) to 20 feet (6.1 meters) in depth will be selected and constructed in accordance with the
following requirements:

Type A 3/4 foot to 1 foot (53° sides)
0.25 m to 0.30 m
Type B 1 foot to 1 foot (45° sides)
0.30m to 0.30m

No benching is allowed for Type C soil.

Benching of the Excavation Walls shall meet the same Safe Slope requirements of 1m
Vertical to1.5m Horizontal.

The maximum height between Benches shall not exceed 1m Vertical, shall not be less than 1
m Horizontal, the minimum (Bench Width).

Spoils shall be placed at least 1m outside of the Zone of Influence.

Note: Simple slope excavations in Type A soil, which are open 24 hours or less (short-term)
and which are 3.5 meters or less in depth, will have a maximum allowable slope of
1/2 H:1V (63°).
Note: Simple slope excavations in Type A soil, which are open 24 hours or less (short-term)
and which are greater than 3.5 meters) in depth will be 3/4 H:1V (53).
Note: No soil classification is required if 0.45 meter to 0.30 meter) (Horizontal to Vertical) or
34° slope is used – refer to Attachment 02. If a 0.30 to 0.45m (Horizontal to Vertical)
34° is not used, a soil classification will be made.

13.0 SAFE SLOPE (BATTER)

The Slope (Batter) of an Excavation shall be at least 1.5m Horizontal to 1m Vertical (45°
degrees measured from Horizontal).

A Slope steeper than 1.5m Horizontal, to 1m Vertical may be used, where a civil engineer,
suitable experienced in the soil and rock slope design and construction, has assessed the
soil conditions.

Examination of site conditions by a certified civil is necessary to determine all safe slopes for
excavations. safe slopes depend on the height of the face, soil type, geological conditions,
the moisture content of the soil and any surcharge loads. soil moisture content and
geological conditions may change as excavation progresses, causing previously safe slopes
to become hazardous.

Spoils shall be placed at least 1m from the edge of the Batter/Safe Slope.

14.0 SHORING

Excavations greater than or equal to 1 m deep are particularly hazardous and must be
shored unless:
 The face is cut back to a Safe Slope, and the material in the face will remain stable
under anticipated conditions of work and weather; or
 Shoring is impracticable or unreasonable, and a Certified civil engineer suitably
experienced in the soil and rock slope design and construction has certified that
adequate precautions have been taken; or
 No one will be entering the Excavation.
Protection of personnel in excavations shallower than 1.5m shall also be provided where
hazardous ground movement may be expected, or where the activities to be conducted
in the trench requires the workers to crouch down.

15.0 EXCEPTIONS

Exceptions to the requirement for protective systems include the following:


 Excavations that are made entirely in stable rock (natural solid mineral matter that can
be excavated with vertical sides and remain intact while exposed: this will be classified
by a Competent Person).
 Excavations that are less than 1.5 meters in depth where examination of the ground by a
Competent Person reveals no indication of a potential cave-in.

16.0 ACCESS / EGRESS

In trenches 1.2 meters or more in depth, ladders, steps, ramps, or other safe means of
access/egress will be provided and located no more than 7.5 meters apart laterally. If ladders
are used, the ladder will extend 1 meter above the original surface of the ground and will be
secured.

Structural ramps that are used solely by employees as a means of access or egress from
excavations will be designed by a competent person. Structural ramps used for access or
egress of equipment will be designed by a competent person qualified in structural design,
and will be constructed in accordance with the design.

Ramps and runways constructed of two or more structural members will have the structural
members connected together to prevent displacement. Structural members used for ramps
and runways will be of uniform thickness.

Ladders shall be properly secured and extend at least 1m above the surface of the
excavation or Intermediate Landing.

Cleats or other appropriate means used to connect runway structural members will be
attached to the bottom of the runway or will be attached in a manner to prevent tripping.
Structural ramps used in lieu of steps will be provided with cleats or other surface treatments
on the top surface to prevent slipping.

Walkways, ramps, or bridges with standard guardrails will be provided at all excavations and
trenches where employees may cross over. The crossing structure will be made of tightly
secured and uniformly sized planking or other similar material. Fall Protection will be
provided where walkways are 1.8 m or more above lower levels.

Where personnel may fall 2 meters or more, gangways shall be fitted with guardrails and toe
boards be at least 0.46m wide personnel access and 0.6m wide for materials access.

17.0 BARRICADES

Where Barricades and Signage is required to be erected around any excavation or


penetration works to prevent falls from height, it shall be erected and maintained in
accordance with the requirements.

Vehicles and personnel not working in the immediate vicinity of the Excavation shall be kept
at a safe distance away from the area. Barricades shall be erected to prevent unauthorised
people from entering the Excavation Area or, accidental falls into the Excavation. Barricades
shall also be erected to prevent vehicles or equipment from being located too close to any
Excavation.

NOTE: Coloured warning tape, rope, cones or flasher units DO NOT constitute a Hard
Barricade.

18.0 EQUIPMENT

When mobile equipment is used or permitted to operate adjacent to excavations or trenches,


barricades will be provided. Wells, pits, shafts, trenches, or other similar ground all hazards
will be barricaded or covered.

A Warning System shall be used when Mobile Equipment is operated adjacent to the edge of
an excavation if the equipment operator does not have a clear and direct view of the edge of
the excavation.

The Warning System shall consist of Barricades, and Hand or other Mechanical Signals.

Mobile plant, vehicles or any heavy loads must not approach closer than:
 1.5 m from the edge of the Excavation, which is Battered to a Safe Slope; or
 The edge of the face, if battered to 45 degrees, unless the actual face is specifically
shored to allow for the full effect of the additional load

19.0 EXCAVATED MATERIALS

Employees will be protected from excavated or other materials and equipment that could
pose a hazard by falling or rolling into excavations. Protection will be provided by placing and
keeping such materials or equipment at least 1 meter from the edge of the excavation, by the
use of retaining devices that are sufficient to prevent materials or equipment from falling or
rolling into excavations, or by combination of both.

20.0 DUST

Airborne dust will be kept to a minimum by the use of water or other means.
