01 Executive Summary:
02 Introduction:
The cable replacement in Camp-4 & 5 requires D.G. Sets to provide temporary power
supply for the affected residential customers, the aforesaid D.G. Set Cater Pillar
Brand & F.G Wilson with 513KVA & 635 KVA were rented by STICCO from MJI supplier
associated with 5SXF303-3 & 5SXF304-2 respectively.
03 Incident Description:
1) The D.G. Set (513KVA) associated with existing transformer 5SXF303-3 (250KVA)
was stopped at around 04:00 pm (06-Sept-2020) resulting power interruption in
camp-5 residential customers. The temporary power supply was restored back
at around 4:30 pm on the same day after replacement of faulty D.G set.
Given Below are the detail of affected D.G set:
Capacity – 513 KVA

2) The D.G. Set (167 KVA) associated with 5SXF304-2 was stopped two time
a) First time at around 7:20 am result in interruption of supply and restored
back at 8.02 am after replacement of damage speed sensor (07-Sept-2020).
b) Second time the tripping was happened at 1:30 am, 1:55am & 2:15 am
result in interruption of supply and finally rectify & restore at 3.00 am after
cleaning the moisture of speed sensor (08-Sept-2020).

Preliminary check was conducted based on checklist below shows all normal.
5.1. Analysis
Summary of key findings.
1) The aforesaid affected D.G was re-start again & again many times due to
frequent tripping (the expected problem in generators alternator & AVR) before
the replacement existing 5SXF303-3 (250KVA).
2) The D/G Set associated to 5SXF304-2, the speed sensor was found damage.
Immediate causes.
1) The D.G. Set associated with 5SXF303-3 had voltage fluctuations & frequent
tripping due to poor maintenance or because it is an old model.
2) The D.G. Set associated with 5SXF304-2 was abruptly stopped due to poor
maintenance or because it is an old model.
5.2 Conclusions:
The D.G. set being used provide temporary at Camp-5 was found to be unreliable as
mentioned in the above findings and causes.
06 Mitigation proposal:
1. STICCO will hire the New D.Gs set on rental basis from the reputed D.Gs
supplier of the Market (i.e. Zahid tractor & Al-Taaqa).
2. The pre-qualification of the D.G supplier shall be submitted along with the
technical data sheet (e.g. kVA ratings, Year Model, manufacturer, etc.) to
Client for review and approval.
3. STICCO to purchase new D. Gs set of appropriate ratings rather than using the
rental one.
4. STICCO will carefully observe the DGs during operation & shall rectify such type
of problem on urgent basis, if occurred.
5. STICCO will increase the frequency of the visits at each & every installed D.Gs.
The above mitigation will help us in reducing risk of undesirable tripping from such
type of occurrence.
