Good Morning everyone!

Welcome to ISAT-TESDA, your Accredited Assessment Center for Computer System Servicing NC II.
My name is Paula Joyce P. Balayan, I am an Accredited Competency Assessor for Computer System
Servicing NC II.

This is my I.D as a proof that I am Accredited Assessor

I would like to acknowledge the presence of our Assessment Focal Person, Ms. Femia V. Calacsan, to our
Assessment Center Manager, Mr. Edwin P. Madarang and to our TESDA Representative from TESDA
Provincial Office, Mr. Armando Daquiog.

Ma’am/Sir a pleasant Morning

My dear candidates, don’t you worry about their presence, they are here to oversee the conduct of
competency assessment and to check if the conduct of assessment is in accordance with the prescribed
competency assessment methodology and procedures.

Take note: they will not assess you, I’ll repeat they will not assess you.

Before the assessment start, let me first check the attendance. As I call your name, please present to me
your admission slip and affix your signature at the attendance sheet
( let the candidate sign in the attendance sheet and compare signature of candidates
“admission slip to attendance sheet”)

I have here with me your accomplished application form and self-assessment guide. It is an
indication that you know and can perform all the unit of competencies in accordance with the prescribed
standards and procedures.

CONTEXT AND PURPOSE OF ASSESSMENT

Today, I am going to administer the Competency Assessment for Computer System Servicing NC
II. Of course, the purpose of this assessment is to determine your level of competency, for certification and
also for employment purposes. After you passed this assessment you are now a certified Computer System
Servicing. This qualification composed of Basic, Common and Core competencies. For the Core level we
have seven (4) competencies to be assess namely:

1. INSTALL AND CONFIGURE COMPUTER SYSTEM


2. SET-UP COMPUTER NETWORKS
3. SET-UP COMPUTER SERVERS
4. MAINTAIN AND REPAIR COMPUTER SYSTEM AND NETWORKS

ASSESSMENT PROCEDURES TO BE UNDERTAKEN

I am going to collect evidences through the assessment methods of:

 Demonstration with oral questioning


 Interview

After your demonstration, later on you will undergo for an oral interview. This is for us to measure your
underpinning knowledge and skills in this qualification.

ALLOWABLE/REASONABLE ADJUSTMENTS APPLICABLE

 For your personal needs, if you feel hungry you can take your snacks or meals at the canteen,
located at the outside of the assessment room
 The comfort room is located inside the assessment area, located at the left back corner of the
assessment room
 If you are not feeling well, you can go to our clinic. It is located at the back of the cookery room.
However, later on you can have your necessities first because once the assessment started you are no
longer be allowed to leave the premises.
 May I ask you my dear candidates if any one of you have physical difficulties? If none, then very
good. In case that there is anyone here, just approach me so that we can address it immediately.

EXPLAIN THE ISSUANCE OF NC II / COC

 For the issuance of National Certificate or what we call the COC (Certificate of Completion), you
can claim it at TESDA Provincial Office located at Alibagu City of Ilagan, Isabela within three (3)
working days after the assessment;
 And after this assessment that you are found to be not yet competent, you don’t have to worry
because you can be re-assessed anytime at your own convenience. In this qualification, you are only
be re-assessed on a certain unit of competency that you are found not yet competent;

CONFIDENTIALITY OF ASSESSMENT OUTCOME

 With regards in the result of the assessment, we observed strict confidentiality, the assessment result
will only be made available to authorized personnel and to the institution where you had undergone
your training.
 In relation to this, the assessment package is kept in a safe place to guarantee its integrity.

DO’S / DON’T’S

 If you have questions, clarifications or queries you may raise your hand so that I can help you
because once the assessment started you are no longer allowed to ask questions.

 May I request everyone, especially you my dear candidates to please refrain from giving away any
form of information that took place during the assessment.

OCCUPATIONAL SAFETY AND HEALTH REQUIREMENTS

 During the actual assessment please observe safety procedures as you perform the different task to
avoid any form of accident.
 In case of accident please call my attention so that we can provide you first-aid. When need arises
our medicine cabinet is located at the left corner of the practical work area.

DISTRIBUTE COPY OF THE SPECIFIC INSTRUCTION TO EACH CANDIDATE

 Do you have any questions or clarifications? If none, I am going to distribute to you the SPECIFIC
INSTRUCTIONS.
 So this is the given instruction. Please read it carefully and if you don’t understand my medium of
instruction, you can call my attention so that I can explain it to you in a way/language you can
understand.

ADMINISTER ASSESSMENT

 Any questions? If none, I will now give it to you your assigned number.

 Ms, Erika Angoluan you will be candidate number one (1)


 Ms. Precious Mae Manzano you will be candidate number two (2)

 Candidates be ready, let’s now proceed to the assessment area. This way……
 So this is the assessment area where you will utilize all the unit of competencies in this qualification.
It’s your decision on what type of competency you are going to do first to finish the given tasks
within the allotted time.
( all the tools, materials/supplies and equipment are already provided inside the assessment area)

SPECIFIC INSTRUCTION

 Given the tools, materials/supplies and equipment you are required to perform the task within 8
hours. Inclusive with the oral interview.
 You need to finish all the task within the allotted time, if not you are considered not yet competent.
 If you failed to pass certain unit of competency in this qualification, you are considered not yet
competent.
 Candidates remember that in performing, you need to wear your appropriate personal protective
equipment and observe safety precautions to prevent accidents.
 During and after your demonstration, I will conduct oral questioning. So be ready.
 Don’t hesitate to ask if you have further clarifications
 Any questions or clarifications? If no more questions you may now start. The time is 9:00 AM.

DEMONSTRATION AND ORAL QUESTIONING

 Let the candidate perform the task and ask follow up questions while they are performing
 Gather evidences by using demonstration checklist

Ms. Erika you are performing hair and scalp treatment. What are the tools and materials needed in
performing the job?
 Chemical cape
 Earpads
 Treatment product
 Clips/clamps
 Bath towel
 Neck strip
 Shower cap
 Headband
 Heating machine
Ms. Precious as I can see, you were draping the client. Why it is important to drape the client before
performing the job?
 To protect the clothing of the client from dirt and stains.

ORAL INTERVIEW

 What are the required tools, materials and equipment needed in performing hair and scalp treatment?
 Why do we need to wear appropriate personal protective equipment in performing a job?
 What are the different types of developer used in performing hair coloring?
 When performing hair bleaching to the client, is it necessary to brush the hair?
 Enumerate the seven unit of competencies in Hairdressing NC II

PROVIDE FEEDBACK ON THE ASSESSMENT FOR COMPETENT

 Ms. Erika Angoluan, how do you feel about the assessment? My observation coincides with your
opinion and I would like to congratulate you because in your performance I found out that you are
COMPETENT, you answered all the questions very well, and you performed the tasks within the
prescribed standard requirements of the qualification. I like the way you focus on what you are
doing, you really mastered your qualification and you always observe safety precautions. However,
in performing you don’t need to rush we just take it slowly but surely. Again, congratulations for a
job well done, you are now certified HAIRDRESSER.

 Please affix your signature to the Rating Sheet and to the Competency Assessment Results Summary
(CARS)
 This CARS will be given to you by our Assessment Center Manager after affixing his signature and
use this CARS in claiming your National Certificate (NC) at TESDA Provincial Office located at
Alibagu, City of Ilagan, Isabela after three (3) working days after the assessment and please bring
with you your passport size picture with white background/ formal attire, one (1) piece only and 50
pesos fee for the processing of your NC. Again, congratulations.

PROVIDE FEEDBACK ON THE ASSESSMENT FOR NOT YET COMPETENT

 Ms. Precious Manzano, how do you feel about the assessment? You did very well on the task, you
did your best but base to the result of my observation you did not meet/perform the prescribed
standard requirements of the qualification. In the interview, you answered some of the questions but
most of the questions are not answered well. Don’t be disappointed on the result, you have
knowledge about your qualification but you lack of experience in one (1) of the competency of this
qualification which is “Perform basic hair perming”. You need to review and practice your
competency qualification further to become better and COMPETENT. However, you can be re-
assessed anytime at your convenience. Don’t you worry because are only be re-assessed on the unit
of competency that you are found not competent.

( Present the rating sheets to the candidate to affix her signature as a proof that he accepts your
assessment decision )

 Please affix your signature to the Rating Sheet and to the Competency Assessment Results Summary
(CARS)

 This CARS will be given to you by our Assessment Center Manager after affixing his signature and
use this CARS in claiming your Certificate of Completion (COC) at TESDA Provincial Office
located at Alibagu, City of Ilagan, Isabela after three (3) working days after the assessment and
please bring with you your passport size picture with white background/ formal attire, one (1) piece
only and 50 pesos fee for the processing of your COC.
 Congratulations and comeback if you feel confident enough to undergo re-assessment.

SUBMISSION OF ASSESSMENT RESULT TO THE ASSESSMENT CENTER MANAGER

 Good Afternoon sir, may I now submit the result of assessment for HAIRDRESSING NC II. So, out
of 10 candidates assessed, there are 9 COMPETENT and 1 NOT YET COMPETENT.
