Campus, do hereby permit him to join and participate in the USCF Strategic Planning to be held
on August 24-25, 2023, at San Juan, La Union, Philippines.

It is hereby understood that I will remain legally responsible for any personal actions taken by
my son. The undersigned likewise affirms to concur and agree with the rules, policies and
regulations being implemented by the concerned organizers and agrees to waive any
responsibility on the part of the University and/or the representative/s, in case of an untoward
incident that may happen to my child during the duration of the aforementioned event.
