    By virtue of power vested on me, I hereby appoint you as BARANGAY


    HEALTH WORKER of Barangay West Dirita, San Antonio, Zambales, effective
    upon assumption, with authority to discharge the functions and duties vested in
    the said office in accordance with laws.
    This appointment shall be enforced and valid sooner until revoked by the
    undersigned subject to existing laws, rules and regulations.
    Issued this 2nd day of September, 2023 at the Barangay West Dirita,
