AFFIDAVIT OF LOSS

I, DARNEL GAY LUMOR ABELLANO, of legal age, Filipino, single,


and a resident of Meridienme Home Corporation, Upper Quijada,
Guadalupe, Cebu City, Philippines, after being duly sworn to in accordance
with law, do hereby depose and state, that:

1. I am a user/subscriber of a TNT Telecom pre-paid SIM Card with


number 0948-7876545;

2. I have been using the aforementioned SIM Card and the same
contact number for approximately seven (7) years already. In fact, my
GCash account is registered under the said number;

3. On December 8, 2022, I discovered that I lost the wallet where the


subject SIM Card was kept;

4. I exerted diligent efforts to find the wallet and the TNT SIM Card
containing the number 0948-7876545, but the same could no longer be
found;

5. I am executing this affidavit to attest to the truth of the foregoing; to


support my application for a new prepaid TNT SIM Card with the same
number, 0948-7876545; and for whatever legal purpose this affidavit may
serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 8th of


December, 2022 at Cebu City, Philippines.
