SWIMMING CONSENT FORM

The swimming pool at school will be opened out for students from 17th February 2023 (Friday). In
order for your child to participate in this extracurricular activity, it is essential that you complete and
return this form supplying relevant information. Kindly return this form on or before 20th June 2023
(Tuesday).

Note:

• The child will be trained by a female trainer along with a Lifeguard.


• Parents should provide the child with appropriate swimsuits (Towel, swimming cap, goggle
Girls – Single piece swim suit with full/half sleeves knee length shorts,
Boys – Shirt and knee length shorts) and a bag to accommodate the necessities. Sleeveless
swim suits and shorts above knee length are not allowed.
• Children will not be allowed to use the pool if the swim suits are not appropriate.
• If the child is unwell, kindly inform the teacher in prior to reduce the spread of infection.
The school also reserves rights to stop the child from using the pool in case of allergies and
infectious conditions.
• Slots for the children (separate slots for boys and girls) will be posted in the Google

Classroom.
