INTRODUCTORY PHASE: It establishes the tone for the remainder of the relationship during this
phase. Throughout this initial encounter, the client and nurse closely observe one another and
make judgments about one another's behavior.
EVALUATION: Effective
By clearing fixing her table first, the client demonstrated her willingness to participate and
redirected her attention to the conversation.
By introducing myself, stating my intentions, and addressing the client by name, I was able
to put the client at ease.
By eliciting information about the client's current state through open-ended questions, I
was able to express my concern for her well-being and give the client the sense that she is
in control of the conversation.
The client began to open up and provide lengthy responses that allowed her to express her
concerns completely.
The client was able to articulate goals for the session, indicating an openness to discussing
her feelings.
By starting with coaxing questions, I can earn the client's trust and demonstrate that I
genuinely care about her well-being.

develop insight into her behavior and how she relates herself to other people.
Seeking clarification and gauging the client’s nonverbal response allowed both of us to
create a mutual ground for understanding her thoughts.
Through the use of silence, attentive listening skills, and open hand gestures, the client was
able to pinpoint the source of her concerns.
The client demonstrated trust by soliciting my opinion and inquiring about my perspective,
demonstrating our mutual respect for one another as unique individuals.
The client was able to accept and identify her feelings through the technique of reflecting.
Despite her apparent reluctance, she made an effort to process and verbalize her thoughts.
My goal was to demonstrate to her that her feelings are legitimate and that life is not about

openly and honestly during the termination phase. This discussion about termination should
begin prior to the termination interview.
Evaluation: Effective
We were able to establish caring, harmonious relationships.
Collaboration was encouraged, and the conversation was smoothly summarized and
concluded.
The client was able to glean meaningful excerpts from the conversation.
Both participants left the conversation at ease and free of anxiety or dependence.
OVERALL EVALUATION:
The goal has been accomplished because the client has demonstrated signs of growing insight,
trust, and security along with her newly enhanced ability to speak up and perceive her feelings
as legitimate responses to difficult circumstances. Rather than avoiding confrontation, the
client confronted her thoughts in her head and even made progress by identifying coping
mechanisms that would help her avoid anxiety-provoking situations and thoughts.
Collaboration and the development of an action plan were also suggested at the conclusion of
the conversation, indicating that both the nurse and the client recognize the other's uniqueness
and importance in achieving the stated goals. Similarly, the client's agreement with an
expression of gratitude for the nurse's statements during the working phase demonstrates her
cooperation and participation in her treatment. Additionally, with the proper support and
guidance, the client was able to grasp a clearer perspective. Additionally, she was given the
opportunity to reflect and vent out her thoughts independently, which was complemented by
the student nurse's open-ended questions and gentle coaxing. Both the client and the nurse
were able to communicate their feelings and points of view without misunderstanding one
another. I believe that the client benefited from these exchanges' overall effectiveness. This is
demonstrated by the client's inability to maintain effective eye contact at the start of the
conversation, as well as by her smile and appearance of hope at the session's conclusion. In my
opinion, the most effective therapeutic communication techniques were reflection and open-
ended questions. The client is aware of several factors that are beneficial to her well-being but
is occasionally afraid to participate actively due to her anxiety. Overall, this conversation has
therapeutic value, as evidenced by the client's desire to improve his or her health and well-
being.
Reflection
In summary, the session went smoothly due to both participants' openness and
willingness to communicate. I have discovered that therapeutic communication is critical in
nursing care because it increases client participation and accelerates recovery through
patient education. As a result, this exercise can be replicated with additional clients by
demonstrating situational awareness, maintaining open communication, and catering to
and respecting their unique needs, which will result in appropriate client support.
Individuals have varying levels of experience and beliefs. Therefore, a nurse must exercise
adaptability and flexibility as this could result in more positive outcomes and harmonious
nurse-client relationships. On the contrary, some areas for improvement could include
asking more personalized questions, paying closer attention to non-verbal patient cues, and
reducing barriers to therapeutic communication.
Analyzation
I had an analysis of the importance of process recording in a nurse-client relationship during
the interview and the creation of the written documentation of the process recording.
Nursing students benefit from process recording because it teaches them how to
conceptualize, organize, communicate with clients, be efficient, and comprehend or
evaluate even the smallest details. The primary goal of the interview during the process
recording is to obtain clarifications and determine whether the conversation and techniques
used are therapeutic or not. Process recording identifies strengths and weaknesses in the
nurse-client relationship and aims to improve the overall conversation.
