Lost Correction of particulars
• For Correction of Particulars attach the original Document/s & attested copy of matric
certificate (SSC).
• Fee must be submitted separately for application Lost and correction at the same time.

• All attested Copies of pass DMCs must be attached.
3. Verification of documents (Transcript/Dmcs/Distinction)
• All attested Copies of pass DMC’s must be attached.
• Fee for Verification through email. (Normal fee Rs.2000 and Urgent fee Rs.4000)

4. NOC for appearing in B.Sc. English Examination only.

Instructions

• With Normal Fee response will be send to the given mailing address through TCS after 15 working Days of
receipt of application in Examinations Department UHS, Lahore.
• With Urgent Fee response will be send to the given mailing address through TCS after 03 working Days of
receipt of application in Examinations Department UHS, Lahore.

Mode of Payment
• No Pay order/ Bank Draft / Cheque is acceptable.
• https://fms.uhs.edu.pk/ (FOR CHALLAN GENERATION)
• The fee must be deposited through online generated challan in any NBP (National Bank of Pakistan) Branch
Online NBP Account # 3001409628 {Sheikh Zayed Medical Complex Branch (Branch Code: 1707),
Lahore} and deposit slip must be attached with this form.
• In case of correction of particulars, the original documents must be submitted.
• In case of migrated student / a combined transcript may be issued, but the original DMCs of parent
university and passed DMCs of IPS (Islamiat and Pak Studies) and Behavioral Sciences must be annexed.
• AJK and RMU students will not submit the DMC of Behavioral Sciences separately for transcript issuance.
