1. Scope

This procedure is applicable to all personnel who include PT. VME and its subsidiary
companies, employees, subcontractor workers, project client site team members, vendors,
suppliers, auditors, potential clients and any others who are required to work/visit our yard on
official businesses.

3. Abbreviations / Definition
HPMT : Head Project Management Team.
HOST : Means PT. VME employees who are tasked to take care of the visitors / suppliers /
auditors / vendors, etc whilst they are in the yard or company premises.
KTP : Indonesia Identification Card.
IDR : Indonesian Rupiah.

4. Responsibilities
4.1. Safety Induction Training will be conducted by HSE Department for all those who are required
to work in PT. VME. Any waiver for short period access to the work yard within PT. VME
premises shall be on a case by case basis.
4.2. For those who will be carrying out any work within the yard / company premises for a
considerable short period of time of 7 days, it’s can be extended to 7 days only. A short safety
briefing can be conducted by the PT. VME host. These people shall be given a copy of PT.
VME HSE Information for Visitors. In such case, the attendance record (VME-HSE-P16-
APPENDIX 003) of such briefing will be taken by the host and submit to HSE Department in
exchange of Visitor Badges.
4.3. The host is to ensure that these people are guided throughout the duration in the yard.
4.4. Details of the arrangement and co-ordination of the HSE Induction program is tabulated as
follows:

HPMT and / or b. Copy of Passport (For expatriate only)

5. Safety Induction Pass

5.1. All attendees who have attended the full Safety Induction training will be issued with Safety
Induction Sticker and badge at the end of the induction.
5.2. The Safety induction sticker and badge consist of for colors each color will be explained as
follow:
5.3. The safety induction sticker and badge will be valid until last of a project. Thereafter, refresher
Safety Induction Training will be conducted every next project, a new sticker and badge will be
issued.
5.4. For visitors, host will make a formal request to HR Department / Security Department on the
number of Visitor Badges he / she needs.
5.5. A safety induction badge must be returned to the HSE Department if:

▪ The workers in case of resignation or termination.

▪ The project has been completed and did not participate in the next project.

5.6. The visitor badge must be returned to Security Department by visitors who will leave the area
of PT. VME Process.

STICKER AS MENTIONED ABOVE IS NON-TRANSFERRABLE AND NO PERSON SHALL

ENTER PT. VME PROCESS WORKPLACE WITHOUT VALID HSE INDUCTION LABEL /
VISITOR BADGES AND BADGES SHALL BE PROPERLY CLIPPED ON FOR EASY
IDENTIFICATION OF PERSONNEL.

6. Badges Administration
6.1. For genuine wear and tear of the badge, replacement will be free of charge.
6.2. For lost badge, IDR 50,000 for each replacement will be charged.

6.3. Project Contract Manager will be informed and the amount will be deducted from the progress
payment to the Sub-Contractor concerned.

Please be informed that the new schedule for Company Safety Induction Class are as follows:

1. HSE Induction class will be held every Tuesday, Thursday and Saturday except Public
Holiday.

2. The venue for the Safety Induction Class will be held in the VME Conference Room.

3. Please ensure that all participants for the Safety Induction Class are to report for
registration, in the HSE Department, by 8:30 AM.

4. The Safety Induction Class will commence at 10:00 AM. Late-comers will not be
entertained.

5. Safety Induction Class booking must reach the HSE Department one day in advance (by
3:00 PM). Late submission will not be entertained, so plan wisely in advance to ensure
that Safety Induction Class does not exceed the maximum seating arrangements.

6. All participants must be accompanied by Supervisor / Foreman in charge. This is to ensure

and facilitate the smooth running of the class.

7. All Safety Induction Class booking must use the prescribed Safety Induction Class Booking
Form and shall be clearly filled up and submitted in the tray provided via Company
Receptionist (located at the Company Main Office). No filling of the Safety Induction Class
Booking Form is allowed in the HSE Department.

8. Please refer to Company OSH Management System Procedure VME-HSE-P-10 : Safety

Induction for more information and clarification.

Your kind and full cooperation in this matter is very much appreciated.

pertaining to the above mentioned.
