Identify and describe outcome data (achievement, attendance or discipline) that serves as basis
for goal:
To increase attendance among homeless students. To do this, there will be weekly 30-minute
individual counseling for 5 students. The school counselor will also meet with parents in-person
to provide families with resources and discuss a positive behavior plan to increase student
attendance. The students and school counselor will have an attendance sheet to monitor their
progress.

strategies/interventions will be delivered and assessed.

Analyze data related to the goal from supplemental data surveys such as school climate
surveys. Talk with stakeholders (parents, teachers, students, administrators, etc.), to gain
insight about possible factors contributing to identified problems or issues. Summarize
insights/comments from the groups below.
School counselors report: There are a total of 42 students who are homeless (18.9% of student
total). We need to provide more resources for families and help students have housing.
Teachers report: Many students come to class with no classroom supplies and teachers do not
have a classroom budget big enough to support student needs.
Administrators report: Attendance has dropped ever since COVID-19 for all students, and it has
affected vulnerable students the most. We want to increase attendance rates at school.

Convert the student learning objectives to a Likert-scale measure and/or brief answer
assessment.
