This document is a school forms checking report (SFCR) that examines learners' records for inconsistencies. It finds that of the 6 grade 1 records examined for learner LOVE, 1 had an incomplete supporting document. The inconsistency was for learner PUNZALAN, CHESKA. No other inconsistencies were found. There were no records of transferred in learners listed in Tables 3 or 4. The accuracy of the examined records was calculated to be 83%.

1. For Table 1 and Table 2, do not include learners who transferred from private school with pending transfer of form 137 or SF10
2. Table 2 is a breakdown in the data of column 7, 8, 9 of table 1.
3. For Table 3, include all issues of transfers including those learners with pending transfer of form 137 from private school.
It is the breakdown of table 3 in SFCR to be accomplished by the School Head.
4. For Table 4, list of all learners with pending transfer of SF10 (form 137) both from public and private schools with reference to table 3.
5. Add additional rows if needed in table 3 and 4.
