1. Scope of work
Electro Fibre ELECTRICAL is a Civil construction company whose core business is the construction of
Fibre Optic Cable routes i.e. Aerial work, hauling, splicing, blowing fibre and building manholes ,
provision of services to the telecommunications industry. We will create and maintain the highest
standards of Occupational health and safety during the execution of all Electro Fibre operations.
2. H&S Scope
A. Covers the legal requirements of the occupational health and safety ACT 85 of 1993 and its
relevant regulations.
B. Outlines parameters which efficient and effective SHE performances can be optimized within the
organisation.
C. Will assist in the realisation of the organisations vision to become globally competitive through
the establishment of world class practises and standards in order to demonstrate that it values
each member of the organisation and our environment.
D. This Plan will be reviewed annually, mono or as requested by the CEO
3. Introduction
This Health and Safety plan has been prepared in accordance with the requirements of the
Construction Regulations (2014) as promulgated in the Occupational Health and Safety Act 85
/1993 & Regulations (OHS Act). This plan will be reviewed, monitored and developed during the
course of the project to ensure its relevance to site circumstances. Compliance to the OHS Act and
Regulations shall not be limited to the specification and definitions contained in this document.
4. DEFINITIONS
Agent
Means a competent person who acts as a representative for a client;
Competent person
competent person" means a person who—
a) has in respect of the work or task to be performed the required knowledge, training and experience
and, where applicable, qualifications, specific to that work or task: Provided that where appropriate
qualifications and training are registered in terms of the provisions of the National Qualification
Framework Act, 2000 (Act No.67 of 2000), those qualifications and that training must be regarded as
the required qualifications and training; and
b) is familiar with the Act and with the applicable regulations made under the Act;
Chief executive officer (section 16.1)
Every chief executive officer shall as far as reasonably practicable ensure that the duties of his
employer as contemplated in this act, are properly discharged
Assistant to the chief executive officer (section 16.2)
Without derogating from his responsibility or liability in terms of subsection (1), a chief executive officer
may assign any duty contemplated in the said subsection, to any person under his control, which
person shall act subject to the control and directions of the chief executive officer.
Construction manager
means a competent person responsible for the management of the physical construction processes and
the coordination, administration and management of resources on a construction site;
Construction site
means a work place where construction work is being performed;
Construction supervisor
means a competent person responsible for supervising construction activities on a construction site;

Construction vehicle
means a vehicle used as a means of conveyance for transporting persons or material, or persons and
material, on and off the construction site for the purposes of performing construction work;
Construction work
means any work in connection with—
a) the construction, erection, alteration, renovation, repair, demolition or dismantling of or addition
to a building or any similar structure; or
b) b) the construction, erection, maintenance, demolition or dismantling of any bridge, dam, canal,
road, railway, runway, sewer or water reticulation system; or the moving of earth, clearing of
land, the making of excavation, piling, or any similar civil engineering structure or type of work;
Contractor
means an employer who performs construction work;
Hazard Identification
Means the identification and documenting of existing or expected hazards to the health and safety of
persons, which are normally associated with the type of construction work being executed or to be
executed;
Health and Safety File
means a file, or other record containing the information in writing required by these Regulations;
Health and Safety Plan
means a site, activity or project specific documented plan in accordance with the client's health and
safety specification;
Health and Safety Specification
means a site, activity or project specific document prepared by the client pertaining to all health and
safety requirements related to construction work
Method Statement
Means a document detailing the key activities to be performed in order to reduce as reasonably as
practicable the hazards identified in any risk assessment;
Principal Contractor
means an employer appointed by the client to perform construction work;
Risk Assessment
(1) A contractor must, before the commencement of any construction work and during such construction
work, have risk assessments performed by a competent person appointed in writing, which risk
assessments form part of the health and safety plan to be applied on the site, and must include—
(a) the identification of the risks and hazards to which persons may be exposed to;
(b) an analysis and evaluation of the risks and hazards identified based on a documented method;
(c) a documented plan and applicable safe work procedures to mitigate, reduce or control the risks and
hazards that have been identified;
(d) a monitoring plan; and
(e) a review plan.
(2) A contractor must ensure that as far as is reasonably practicable, ergonomic related hazards are
analysed, evaluated and addressed in a risk assessment.
(3) A contractor must ensure that all employees under his or her control are informed, instructed and
trained by a competent person regarding any hazard and the related work procedures and or control
measures before any work commences, and thereafter at the times determined in the risk assessment
monitoring and review plan of the relevant site
(4) A principal contractor must ensure that all contractors are informed regarding any hazard that is
stipulated in the risk assessment before any work commences, and thereafter at the times that may be
determined in the risk assessment monitoring and review plan of the relevant site.

(5) A contractor must consult with the health and safety committee or, if no health and safety committee
exists, with a representative trade union or representative group of employees, on the monitoring and
review of the risk assessments of the relevant site.
(6) A contractor must ensure that copies of the risk assessments of the relevant site are available on
site for inspection by an inspector, the client, the client’s agent, any contractor, any employee, a
representative trade union, a health and safety representative or any member of the health and safety
committee.
(7) A contractor must review the relevant risk assessment— (a) where changes are effected to the
design and or construction that result in a change to the risk profile; or (b) when an incident has
occurred.

5. APPOINTMENTS
Electro Fibre shall appoint employees which are relevant to the field of work they are required to do.

Electro Fibre will ensure that all appointment letters are signed. Electro Fibre appointment letters are to
be kept in the health and safety site file.
6. RISK ASSESSMENT
Electro Fibre will appoint in writing a competent person appointed to conduct a risk assessment for the
scope of work to be performed at the Clients Premises and it shall include at least the following:
➢ The identification of the risks and hazards to which persons may be exposed to;
➢ The analysis and evaluation of the risks and hazards identified;
➢ A documented plan of safe work procedures to mitigate, reduce or control the risks and hazards
that have been identified; ➢ A monitoring plan; and ➢ A review plan.

The risk assessment will be made available to all staff at all times on the construction site. All staff will
be made aware of the hazards and risks on the site as well as the measures that are required to
mitigate or eliminate the risks.
Risk assessment will be reviewed whenever there is an incident or change in the scope of work.
Safe working methods, procedures and processors shall be documented, accessible and implemented
to ensure that work is performed safely.
8. HEALTH AND SAFETY AWARENESS
Electro Fibre shall ensure that its workers are trained on all the following documents to ensure they
are aware of the hazards and risks that surround them on site.

b. Tool box Talks
c. Electro Fibre Health and safety policy
d. Health and safety induction
e. PPE/PPC policy
.
9. AERIAL WORK/WORKING AT HEIGHTS
All aerial work conducted by Electro Fibre will be done under close supervision of the appointed
supervisor in the site file.
All equipment used for the AERIAL WORK will be inspected and documented on the prescribed
checklists that are ladder checklist, safety belt/harness checklist, all deviations will be noted and the
specific equipment with the deviations will be removed from site.
Only workers that have received training in working at heights will be allowed to work at heights and
Be in possession of a competency certificate.
10. EXCAVATIONS
Electro Fibre will ensure that all excavation work is done under the supervision of a competent
person appointed in writing.
Electro Fibre will evaluate, as far as reasonable practical the stability of the ground and scanning for
underground services before its works commences.
Electro Fibre will ensure that suitable steps are taken in order to prevent any person from being
buried, trapped by a fall or dislodgment of material in an excavation.
Electro Fibre will ensure the following:
a. That no soil/spoil, load, material, plant or equipment will be stored at the edge of any
excavation where it may cause its collapse and consequently endangers the safety of
any person.
b. Excavation spill will be placed at a minimum 500mm away from trench where reasonable
possible.
c. That Electro Fibre will ascertain as far as reasonable practicable the location and nature
of electricity, water, gas or other similar services which may in anyway be affected by the
work to be performed. Necessary steps will be taken to render the circumstances safe for
all persons involved. The excavation supervisor
d. will ensure every excavation which is accessible to the public or which is adjacent to
public roads or thoroughfares, or whereby the safety of persons may be endangered to
be adequately protected by a barrier or fence of at least one metre in height and as close
to the excavation as practicable.
e. will ensure convenient and safe means of access to be provided to every excavation in
which persons are required to work. Such access will not be further than six meters from
the point where any worker within the excavation is working.
f. Warning signs will be placed next to excavations where persons are working or within the
area of which persons may be working.

g. Barriers, or stop blocks, will be placed to stop construction vehicles from moving too
close to the edge of an excavation and endanger its stability or persons working in the
excavation
h. Excavations will be kept free from toxic or explosive gases.
i. Safe access will be provided across excavation areas to accommodate the public to
enter their properties. Proper barricading at such entrances / exits will also be put in
place.
j. The depths of excavations will be according to the client’s requirements.
k. Pilot holes that will be excavated will be at least 1 metre deep to expose underground
services.

11. CONFINED SPACE ENTRY TO MAN HOLES
The appointed supervisors on site will ensure that man holes are opened using the man hole keys
provided for the man hole
Electro Fibre will ensure that the man holes vent first for about 10 minutes before entering.
Air quality monitors will be used to test the air quality where required to.
Air quality monitors are not kept at Electro Fibres disposal and will be hired until such time the
company can purchase one.
Workers entering man holes will use a full body safety harness and have a life line attached to it and
will be pulled out in case of emergency.
12.1. Splicing:
Only a competent splicing technician shall undertake all splicing processes.
Electro Fibre will ensure that all clients are connected to the service provider within the time frame
given.
Supervisor of machinery to ensure that splicing machine is collaborated and certificate of proof is
available on site file.
12.2. Hauling of duct tubes from manhole to man hole:
Hauling of micro ducts into duct sleeves from man hole to man hole shall be done under the supervision
of an appointed supervisor in writing. Supervisor to ensure that De-coilers and cable drums are used
during the process of hauling at all times.
12.4. Blowing of fibre optic cable from man hole to man hole:
Electro Fibre shall blow fibre optic cable from man hole to man hole using a compressor and fibre optic
blowing machine, this process shall be done under strict supervision and by an appointed compressor
operator.
13. THE USE OF COMPANY VEHICLES
Electro Fibre will ensure that all construction vehicles and mobile plant are operated by a person Who
has received appropriate training, is certified competent and is in possession of proof of
Competency and is authorized in writing to operate those construction vehicles and mobile plant.
All vehicles and mobile plant shall be of an acceptable design and construction. Vehicles and mobile
Plant will be inspected by an appointed person in writing and such reports will be documented on a
Checklist/register and be kept on the site file. The vehicle and mobile plant inspection
Checklist/register shall be done prior to using the vehicle and mobile plant any deviations must be
Reported to the supervisor and be attended to immediately by a competent person to carry out Repairs
etc.

Electro Fibre will ensure that all its mobile plant and vehicles are equipped with an acoustic reverse
alarm where applicable.
Electro Fibre will ensure that all construction vehicles and mobile plant travelling, working or operating
on public roads comply with the requirements of the National Road Act.
14. CONSTRUCTION VEHICLES AND MOBILE PLANT
1. Electro Fibre shall ensure that construction vehicles and mobile plants are of acceptable
design and construction and in serviceable condition;
2. Vehicles and mobile equipment are in good working condition and maintained appropriately;
3. Used in accordance with its design and intention for which they were planned, taking
account the safety and health of employees;
4. All vehicles mounted crane and other similar equipment shall be marked the maximum
working load, hooks with safety latches, number, registered and certificated;
5. Ensure that they are inspected daily by a competent person and that the findings of
inspections are recorded on vehicle and motorised equipment checklists;
6. Ensure that attached material and equipment are secured to prevent them from falling;
7. Ensure that operators are trained and certified competent and are authorised to operate
designated class of vehicle and or machinery;
8. Employees shall not be transported in trucks or pick up vans that are not certified to carry
more than 3 passengers as per the license disk.
9. Employees will use public transport to get to site and or depending on the location of the site,
Electro Fibre shall provide transportation to its employees in a form of a bus or taxi.
10. Vehicles and mobile equipment shall only be driven on dedicated routes or travelling ways
on construction sites.
11. Contractor / Sub-contractor vehicles shall be identified by displaying the name of contractor
on a sticker or magnetic sticker on either side of the vehicle.
12. The driver of the vehicle shall not move the vehicles until every employee is seated and
secured with a safety belt.
19. SUPERVISION
Electro Fibre shall ensure that all work performed on site is done under strict supervision and that no
unsafe or unhealthy work practices are permitted. In this regard Electro Fibre shall ensure that:
➢ Discipline regarding health, safety and environmental matters is strictly enforced. ➢ Persons are
assigned the responsibility for maintaining housekeeping standards.
➢ Persons are assigned the responsibility for keeping all statutory registers and checklists up to
date.
➢ Regular inspections of all work sites are undertaken by Site Supervisor and Management. The
Client retains the right of access to documentation relating to such inspections.
Electro Fibre shall appoint competent staff to ensure the provisions of the safety plan are met. A full
time supervisor shall be available on site at all times.

4. Loss of wildlife
1. Daily site inspection of vehicles must be carried out and any leaking vehicle taken off
site and repaired immediately;
2. In the event of fuel spills, the contaminated soil must be removed and disposed of at
a licensed hazardous waste facility. Disposal certificates/waybills must be retained
by contractor and stored on the SHE File;
3. Soil that cannot be removed shall be treated on site with appropriate remedial
agents, or the services of an expert must be sourced.
4. Mobile toilets shall be located 100 metres away from water sources and serviced
regularly by a licensed Mandatory. Waybill/service certificates must be retained by
contractor and stored on the Safety File.
5. Hazardous storage and refuelling area must be bunded with an impermeable liner to
protect water sources from contamination.
6. WASTE MANAGEMENT
1. A waste skip shall be provided for collection and storage of construction rubble;
2. Bins must be provided on site for the collection and storage of general waste and the
bins must be emptied regularly;
3. All waste generated on site shall be disposed of at a registered waste disposal
facility and proof of disposal shall be kept on site file
4. Hazardous waste, e.g. oil, fuel, etc. shall be separated from general waste and kept
in separate tamper proof disposal containers and disposed of at approved licensed
Hazardous Landfill sites and proof of disposal records shall be filed.
5. Littering on site is forbidden.
6. All waste shall be disposed of as prescribed above and the use of pits or burning of
waste is forbidden on site.
7. Dust Control
1. Excavation and any handling of erodible material shall be avoided under high wind
conditions;
2. Where possible, soil stockpiles must be located in sheltered areas where they are
not exposed to erosive winds;

3. Dust suppression measures or wet method must be applied, where appropriate.
1. Fuel has a potential for significant environmental impacts associated with the
spillage or leakage of fuel into water sources and soil. The highly flammable nature
of fuel also poses a risk to health and safety of all staff on site.
2. An area for fuel storage and dispensing must be provided and demarcated way from
storm water systems and all water sources (ponds, dams, etc.). All storage tanks
must have adequate bunding. The bund shall be impermeable.
3. All vehicles and plant equipment shall be refuelled at a designated refuelling area,
designed to prevent spills and pollution of soil and water sources.

4. • In any medical case, the incident / accident must be reported to the Client for
accident / incident investigation purposes.
1. Electro Fibre shall ensure a document Traffic Management Plan is developed detailing the
procedures to be implemented on site to manage and ensure the safety of all employees, public
and passing motorists. The documented plan must be approved by the Clients representative
2. In the event of work being carried out on main roadways the Traffic Management Plan is to be
co-signed and approved by the local metro traffic department and the Clients.
3. All site vehicles are to be positioned on site in a manner as not to cause any undue
inconvenience to passing motorists or pedestrians.
4. Any work being carried out of the pedestrian walkways that causes a disruption to the natural
flow of pedestrian traffic an alternative safe means of passage is to be constructed and clearly
demarcated.
5. Access to affected working areas is to be restricted and fully barricaded to prevent unauthorised
entry.
6. Sufficient caution signage is to be posted to caution public of the activities taking place and
precautions to follow.
7. Public liability insurance cover is to be in place and a copy of the policy is to be available in the
Safety File.
8. Electro Fibre appointed supervisors are to ensure the above mentioned points have all been
implemented prior to the commencement and any activities and ensure compliance to the
preapproved Traffic Management Plan.
9. Implement any other requirement requested by the relevant metro traffic department or on
request by a Clients representative
28. STORAGE OF FLAMMABLE SUBSTANCES
There will be no need to have a lockable flammable store as re-fuelling of compressor will be done at
the Head Office. There will be a 5lt jerry can on site for containing petrol for the compactor.
29.CLEANLINESS AND HOUSEKEEPING
Electro Fibre shall ensure that it always maintains a clean and sound environment while performing
tasks.
The appointed housekeeping supervisor is to ensure that regular inspections of the workplace is done
and is kept neat and tidy.
Appointed supervisor to ensure that everything has a place and is in its place always packed neatly and
stored in such a way that it causes no harm to employees or public and ensure that it causes no
obstruction to any emergency equipment or walkways.
Clean drinking water shall always be provided to all employees and be available to all employees .

Portable toilets will be hired from Sani-Tec and cleaned by them every 2 nd day to ensure that the toilets
are always clean and hygienic.
30. INTOXICATING LIQUOR, HABIT-FORMING DRUGS OR FIREARMS
Under no circumstance will any employee be allowed on the work site under the influence of alcohol,
drugs and or in possession of any weapon. If found to be under the influence of alcohol or drugs worker
will be dismissed from site and further disciplinary will be taken by Electro Fibre on the employee.
Employees to ensure they inform the supervisor or manager in charge of any medication that they are
prescribed to take.
31. WELFARE FACILITIES
Electro Fibre will ensure that each site has an adequate amount of portable toilets on site, Toilets
will be situated not more than 150 metres from employees. The portable toilet will be Inspected daily
and record will be kept on the site file.
32. LIGHTING
Electro Fibre will provide flood lights for workers at night to ensure that there is adequate lighting on the
work site.
33. NOISE
All machine operators will wear ear protection when operating machinery such as Jumping jack
compactors, Breakers, Compressor and Road saw cutter.

36. HAND TOOLS
The appointed hand tool inspector will be responsible to check hand tools daily and record must be kept
of these inspections:
Hand tools must always be in good working order.
When not in use, hand tools must be safely stored.
38. INSPECTIONS
1. Schedule site inspection has already been defined and will mean the application of
an EHS Site Inspection at the initiation of installation activities at each site and one
monthly EHS Inspection for each live site. These inspections will be co-ordinated with
the Project Manager and will cover 100% of sites that are part of this SOW. Sites with
perceived higher risk will be prioritized for EHS Inspections.
2. Below are lists of checklists that are task specific. These checklists will be placed in a
checklist file and be handed to the responsible supervisors on site. All completed
checklists will be filed and kept on site for auditing and inspection purposes.

Electro Fibre shall ensure that all the machinery or equipment it will utilize on the Clients premises is of
sound order and fit for the purpose for which it is intended, and that it complies with the requirements of
the Occupational Health and Safety Act.
Electro Fibre will be responsible for the removal of any equipment, which is not to the prescribed safety
standards. The Client will not be held responsible for any time lost due to the above.
Electro Fibre and his responsible persons shall ensure that:
➢ All machinery is suitable, operated and maintained to prevent hazardous or potentially
hazardous conditions or circumstances.
➢ All machinery is suitably guarded by means of insulation, guarding and inspection.
➢ Safety equipment is in good working order and is properly used.
➢ The quality of materials and safety equipment used is suitable for purpose intended.
Electro Fibre will produce a "letter of good standing" from the Compensation Commissioner prior to the
commencement of any contract work on the premises.
1. Induction for all Employees by Electro Fibre before commencing any work on site. The induction
shall cover SHE requirements of the specification.
2. The contractor shall conduct daily risk assessments and tool box talks before the start of each task
where workers shall be alerted to particular environmental concerns associated with their tasks for
the day and the area where work will be carried out.
