Definitions
• Safety: Level of acceptable risk from undesired unplanned, uncontrolled
event (ex. hazard) that can cause adverse effects to personnel,
equipment, environment and property within a defined system
• Safety can be expressed as: a measure of risk level associated with the
operation of a system
• Hazard: Something that has the potential to cause human injury, ill
health and/or damage to property (machinery and equipment) or to the
Environment.
• Risk: The combination of the probability of occurrence of the hazard and
the severity of its effects, i.e.
RISK = Probability x Severity
• Probability = Loss event / unit of time or activity
• Severity = Loss / loss event
• Risk = Expected loss / unit time or activity

1 Can the employee be struck by or contacted by anything while doing this job step?
2 Can the employee strike against or make injurious contact with anything?
3 Can the employee be caught in, on or between anything?
4 Can the employee strain or overexert?
5 Can the employee slip or trip on anything. Can he fall in any way?
Can the employee be exposed to any injurious conditions such as g, heat, fumes,
6 pressure, chemicals, asbestos, etc.?

7 Can the employee injure a fellow employee?

8 Can damage to equipment occur?
9 Can pollution of the environment occur?
When is the task to be executed, could it be carried out at a more appropriate time
10 (e.g. during shutdown)?

11 Where is the task to be performed, could it be carried out in a safer location?

Are there any simultaneous operations which have a significant safety impact on
the task (e.g. other tasks occurring as part of the same work scope, or other work in
12
the adjacent area)?

What hazardous materials are involved (e.g. chemicals, combustibles, inert gas)
13 within the system being worked on or within the area worked in?
14 What hazards are introduced by tools and equipment to be used?
What hazards are introduced by the specific circumstances of the job to be
15 performed (e.g. day / night, exposure to weather, in a shutdown, simultaneous
operations, etc.)?
Are there any location related hazards (e.g. working at height, in confined spaces,
16 below ground, etc.)?
Are there any installation related hazards (e.g. pressurised systems, extreme
17 temperatures, etc.)?
Are there any adjacent areas (e.g. below, above, to the side) on which this task may
18 have an impact?
Are there any safety systems involved whose function / availability may be
19
impaired?
Does the task affect emergency response or (e.g. restricting emergency exits, etc.)

Risk Managment
The overall process of identifying, evaluating, controlling and accepting
risks while following all health, safety and environmental regulations and
best practices.

• Risk assessment: The process of evaluating risk

• Residual risk: An expression of risk that has not been eliminated by design

• Controlled risk: The level of risk after controls are applied

• Risk acceptance: The accepting of risk by the appropriate level of

management

Benefits of Risk Assessment

 Compliance with regulations
 Establishes performance standards
 Standardizes operating procedures
 Creates a training tool

Setup involved in the Risk Assessment

Process
• Setup management structure, allocating appropriate authority,
responsibility and accountability
• Establish assets to be protected and establish risk tolerance limits
• Outline the process/task steps
• Risk Assessment: Identify hazards, their likely occurrence and their worst
credible effects on selected targets i.e. assess/quantify/ prioritize hazard
risk
• Risk Mitigation: Follow the risk reduction hierarchy
• Suggest corrective action for acceptable risk

Approximately one single fatality, major property Hazard(s) will be experienced

damage if hazard is realized
Catastrophic (5) Frequent/ Almost Certain (5)

Numerous fatalities, irrecoverable property Hazard highly likely to occur

Standard Operating Procedure

A standard operating procedure (SOP) is a set of written instructions that describes in detail how
to perform a process or experiment safely and effectively. SOPs may be utilized in laboratory
and non-laboratory operations.

The Lab Supervisor is responsible for providing written Standard Operating Procedures (SOPs)
relevant to health and safety for laboratory activities he/she directs involving hazardous
chemicals. Laboratory personnel working autonomously or performing independent research are
responsible for developing SOPs appropriate for their own work.
Examples of SOP topics
• The specific use of a chemical or class of chemicals in experiment (such as a specific laboratory procedure).

• The generic use specific chemical or class of chemicals with similar hazards (for example, mineral acids).
• A generic procedure (such as distillation) that covers several chemicals.

• Radionuclide use of DNA and RNA labeling.

• Laser operations.

• X-ray diffraction machine operation.

• Use and maintenance of laboratory equipment or instruments.

conditions (temperature, pressure, etc.)

2. Potential Hazards

Work practices to be used that reduce exposure to hazardous exposure (Hand washing, no food/ drinks, restrict lab access,
designated areas, keep doors closed, housekeeping, keeping containers closed, not performing procedure alone, etc.).

5. Personal Protective Equipment (PPE)

Identify the required PPE (Eye protection, Hearing protection, Respiratory protection, Foot, Head protection, Body protection, Fall
protection etc.).
