It is compliant with the Company LOPA procedure and data usage.

The scenarios are well defined and all IEs chosen lead to the
consequence, not different (e.g. small LoC vs large LoC)

IEs and IPLs from HAZOP have been considered for suitability to the
LOPA process

All IPLs are Independent of IEs and other IPLs. (e.g. BPCS function to
appear only once in a threat line)

Response & action to an alarm is independent of the IE and other


IPLs (including the equipment in the operator

There are no CMs or EEs with a value < 0.1

Mitigation IPLs have not been credited


All IEs and Barriers have tag names.

There are no IEs that are without IPLs (eg corrosion, pipe leak,
vehicle impact etc, earthquake) - these are to be excluded.

Actions & recommendations > $25k have been internally discussed

Where barriers are suspected to be not effective. This is clearly


documented in the facilitators comments with clear justification

Operator observation does not appear as an IPL
