    AFFIDAVIT OF ADMISSION OF PATERNITY


    I, _________, of legal age, single, Filipino citizen, and a resident of ____________ after
    having been duly sworn to in accordance with law hereby depose and say:

    1. That I am in a common-law relationship with _________ having met her in


    March ______;

    2. That since then, I have been living with her at ____________, though bereft of
    marriage;

    3. That as a result of said relationship, she conceived and gave birth to a child on
    _______ and we named the child, _______;

    4. That I am the biological father of the child, ______;

    5. That at the time of the birth of said child, I am not legally married to _______, her
    biological mother;

    6. That I hereby admit under oath my paternity of the child, ________;

    7. That the reason that my name did not appear on the Birth Certificate of my child
    is due to the fact that at the time of the registration of her birth, I was not able to
    accompany her mother before the City Civil Registrar, which resulted to the
    refusal of the Civil to allow the entry of my name as the Father of the child as her
    mother cannot produce the requisite marriage certificate/Contract;

    8. That I am executing this affidavit to attest to the truth of the foregoing facts and
    for whatever legal purposes it may serve.
