Reason for Referral

The client was referred to the trainee Clinical Psychologis for the administration of Beck
Depression Inventory for academic purpose and psychological assessment.

Family History

Subject's mother was 45 years old. Her qualification was matric. She was house wife. The
subject had healthy relations with her mother. Subject's father age was 50. His qualification
was also matric. He had own general store. The subject relations with her father was also
healthy. Subject had 5 siblings. I born brother age was 25. His qualification was MIT (Master
of Information Technology). He was married. 2nd born brother age was 24. His qualification
was BSCS (Bachelor of Science in Computer Science). He was unmarried. 4 born sister age
was 19. She studied uptil BS (Hons). She was unmarried. The subject shared heathy relations
been with all of her siblings.

Personal History
The subject birth was normal. All milestone was achieved in the normal age level. Subject
started going to school at 5 years of age. His overall class performance was good. He was an
average student. He took part in co-curricular activities in his school. His hobbies were
watching TV and playing cricket.

Educational History

The subject has done his matriculation from The smart school Sambrial. He has done his
intermediate from Punjab group of colleges Sambrial. Now he is doing is bachelor’s degree
from University of Sialkot.

Test Administration

The test was self administered. The subject filled the questionnaire by his own self. Time
taken by the client was almost 15 minutes.

Test Instructions

This questionnaire consists of 21 groups of statements. Please read each group of statements
carefully. And then pick out the one statement in each group that best describes the way you
have been feeling during the past two weeks, including today. Circle the number beside the
statement you have picked.

Behavior Observation

The subject was neatly dressed up. He was sitting on chair comfortably. He showed
cooperative attitude throughout the test. He was able to maintain eye contact. His way of
talking was polite and sweet.

Quantitative Interpretation

Obtained score Level of Depression

Qualitative Interpretation
The total score obtained is 24 which indicates that the client had moderate level of
depression. The client presented high level of symptomatology associated with depression.
The client respond on item number 3 that as I look back in my past, all I can se is alot of
failures. It means that the client is depressed because he believed that there are many things
in his past which he could not achieve. He also respond in some items that he feel guilty good
time, he expect to be punished, he cry all the time now, he also concerned about his looks that
he do not look good now and getting older day by day, he also lost his weight due to
depression. The client should take therapies and a proper treatment plan from the professional
mental health specialist.
