SCHOOL GOVERNANCE COUNCIL
MEMORANDUM OF AGREEMENT
KNOW ALL MEN BY THESE PRESENTS:
This agreement made and entered into by and between:
The SCHOOL GOVERNANCE COUNCIL OF LINANGKOB NATIONAL HIGH
SCHOOL, a duly recognized organization with postal address at Barangay Linangkob,
Kidapawan City, represented by its Co-Chairperson, Roberta C. Bullanday.
And
The BARANGAY HEALTH WORKERS, a government office existing under the laws of
the Republic of the Philippines, with address at Barangay Linangkob, Kidapawan City,
represented by its president Mrs. Eliticia Cañas; and hereinafter referred to as the
Barangay Health Workers Association.
WITNESSETH, that:
WHEREAS, the School Governance Council of Linangkob National High School is
responsible for the governance and management of the school. The Barangay health
workers is a recognized partner organization representing health workers in the local
community;
WHEREAS, the purpose of this memorandum of agreement is to establish a collaborative
relationship between the SGC and BHW to promote the health and well-being of students
and staff of Linangkob National High School. Both parties acknowledge the importance
of a healthy school environment and the role of health workers in achieving this goal;
NOW, THEREFORE, for and in consideration of the foregoing premises, the SCHOOL
GOVERNANCE COUNCIL and the BARANGAY HELATH WORKERS agree and
stipulate the following:
1. Collaboration and Communication: the SGC and BHW agree to collaborate and
exchange information regularly to address health-related issues affecting the
school community. Both parties will designate representatives who will serve
as points of contact for communication and coordination.
2. Health Promotion: the BHW will provide health Education programs,
workshops, and training sessions for students, parents, and staff on various
health topics as agreed upon by both parties. The SGC will support and
facilitate the implementation of these health education programs within the
school.
3. Health Services: the BHW will assist in organizing and providing health services
such as vaccinations, health check-ups, and referrals to appropriate healthcare
professionals. The SGC will ensure the availability of suitable facilities and
resources to support the provision of these health services within the school
premises.
4. Emergency Preparedness: the SGC and BHW will collaborate in developing
and implementing emergency response plans and protocols to address health
emergencies that may arise within the school. Both parties will conduct regular
drills and exercises to ensure preparedness and enhance the safety of students
and staff.
5. Confidentiality: any information shared between the SGC and BHW, including
personal and health-related data, will be treated as confidential and handled in
accordance with applicable laws and regulations.
6. Duration and Review: this MOA shall be effective from the date first written
above and remain in force for period of three years. The SGC and BHW will
conduct periodic reviews to assess the effectiveness of the collaboration and
may amend this MOA by mutual agreement.
7. Termination: Either party may terminate this MOA by providing a written notice
of termination to the other party with 1-month prior notice. Termination of this
MOA shall not affect any ongoing programs or activities, and both parties shall
fulfill their obligations until the effective termination date.
In case any one or more of the provisions contained herein shall, for any reason, be held
to be invalid, illegal, or unenforceable in any respect, such invalidity, illegality or
unenforceability shall not affect any of the provisions of this agreement. In such event,
the parties shall consult each other as to the manner by which their original intention can
be fulfilled as closely as possible and they will amend this agreement accordingly.
IN WITNESS WHEROF, the parties hereof have signed this agreement on this 20th day
of March 2023, in Linangkob National High School.
