This is an open book examination. It is not invigilated, and you are free to use any learning resources to
which you have access, eg your course notes, or a website, etc.
By submitting this completed assessment for marking, you are declaring it is entirely your own work.
Knowingly claiming work to be your own when it is someone else’s work is malpractice, which carries
severe penalties. This means that you must not collaborate with or copy work from others. Neither should

The examination begins with a scenario to set the scene. You will then need to complete a series of tasks
based on this scenario. Each task will consist of one or more questions.
Your responses to most of these tasks should wholly, or partly, draw on relevant information from the
scenario. The task will clearly state the extent to which this is required.
The marks available are shown in brackets to the right of each question, or part of each question. This will
help guide you to the amount of information required in your response. In general, one mark is given for
each correct technical point that is clearly demonstrated. Avoid writing too little as this will make it
difficult for the Examiner to award marks. Single word answers or lists are unlikely to gain marks as this
would not normally be enough to show understanding or a connection with the scenario.
You are not expected to write more than 3000 words in total.
Try to distribute your time and word count proportionately across all tasks.
SCENARIO
You've just started a new job. You are now in charge of overseeing health and safety at a sizable, busy
retail establishment situated outside of a major city with good access to roads. The store sells hazardous
chemicals, tools, equipment, and other hardware and do-it-yourself (DIY) supplies to the general public
and nearby companies. The company that owns the business operates 100 locations nationwide, including
10 in your neighborhood.
Customers are welcome to examine and purchase things in the main area of the store. A big warehouse
with shelves and racks of merchandise is located behind the store and is accessible through an automatic
door opener. The warehouse is restricted to store employees only. Forklift trucks (FLTs) are used by
warehouse workers to transfer cargo from delivery trucks into the warehouse. The merchandise is
relocated into the main area of the store while it is not open to the public in order to replenish the shelves.
The general Store and Warehouse Manager is whom you report. The following people work in the
warehouse:
- A rota system of four days on and four days off with a total of 20 employees (including 2 shift supervisors)
spread evenly between two 12-hour shifts (08:00 - 20:00 and 20:00 - 08:00).
You have witnessed numerous instances of rule-breaking at the warehouse since beginning your new
work. You have observed merchandise heaped in aisles and obstructing designated paths, for instance. As
they move around the warehouse, employees must avoid several hazards, which forces them to cross
paths with moving vehicles. Your employees have informed you that there are frequently close calls
between FLTs and personnel as well as product collisions that result in spills and damage. None of these
are recorded.
Over the years, numerous injuries have been documented. Most recently, a teenage FLT driver was
involved in a recurrence of a more serious crash. Due to the driver's phone distraction, the brakes were
applied too late, causing the FLT to slide on an oil spill and flip over cargo onto a worker who was passing
by.
The worker's leg was broken in this instance, necessitating immediate hospital care which is 8 kilometers
or 5 miles away from the store. The injured employee is anticipated to miss six weeks of work in order to
heal. In order to file a claim for compensation, the hurt worker is seeking legal advice.

In the warehouse, there is a high rate of employee turnover. There are no worker representatives for
health and safety. You were informed by warehouse employees that they frequently complained to
management about the working environment. Management doesn't frequently appear in the warehouse.
There are no documented records of complaints to be found.
You have made an effort to persuade the general Store and Warehouse Manager that improvements in
health and safety are required at the warehouse. You are informed that there isn't any money available
for "that kind of thing" and that even if there was, it would have a negative impact on corporate
operations.
You were visited by a labor inspector who has issued an official order requiring workplace improvements
to improve the health and safety of the employees as a result of the most recent FLT crash. The inspector
believes it is only a matter of time until other workers at the warehouse suffer major injuries or even pass
away. The written risk assessments are too vague and do not accurately reflect the dangers present in the
warehouse, the Inspector further noted. At their subsequent visit, the Inspector wishes to observe a more
effective health and safety management system.
You and the Inspector have spoken about enhancing the warehouse's health and safety. Barriers,
pedestrian walkways, defined crossing points, and separate entrances for workers and FLTs are all part of
the suggested method for separating FLTs and workers. Additionally, you inform the Inspector that you
will compare internal and external health and safety performance.
