LICENSURE EXAMINATION FOR TEACHERS / VARIOUS BOARDS


Guide for Proctors:
1. Report on or before 6:00 AM to the Attendance Supervisor for your Room Assignment.
2. Get your Plastic Envelope (Room Watcher Kit) and proceed to your Room Assignment.
3. In the Room where you are assigned:
3.1 Arrange the chairs in 3 rows (start number 1 on the left side and in snake type, rows may vary on the space
of the classroom and do not forget to occupy the space of the room) and number the chairs at the back of
portion of the chair.
4. Do the board work (facing on the board)

5. Check the forms and count the ID/As Sheets if it’s enough for the number of examinees in your room (refer to
the master list). If it’s lacking, refer to your Floor Supervisor. Take note of the serial Nos.
6. At 6:30 AM, start admitting examinees whose names are found in the computerized list (master list) (Vacant
the seats for the examinees who are not yet around or absent).
6.1 Determine the identity of the examinee base on their NOA and PERRC, and then sign below on the picture of
their PERRC. (To avoid impostor)
6.2The other proctor will guide the examinees to their seats basing on their number in the master list.
6.3 Things allowed inside the classroom: (Please refer on the Board Examination Program)
a. transparent envelope d. calculator (only SECONDARY – MATH can use and
b. brown envelope, in their MAJOR subject only)
c. pencils, pens, sharpener, e. Notice Admission
6.4 Any kind of Gadgets, Review Materials are strictly prohibited inside the classrooms.
6.5 Collect the following:
a. RT – PCR Test or Certificate of Quarantine c. Annex B – Health Declaration Checklist
b. Annex A – Informed Consent

IF EXAMINEES ARE ALL SEATED :( Recheck again the examinees if they seated on the seats intended for them, refer on
the master list.)
7. Ask them to write their name, room no., seat no., and their application no. on the back of their brown
envelope.(Empty the brown envelope that is intended for AS/ID Sheets only)

8. At 7:00 AM distribute the ID/AS Set accordingly.

9. Let the examinee check the following on their set.


9.1 No. of Sheets (Elementary – 3 Pages = 1 ID sheets and 2 Ans. Sheets)
(Secondary– 4 Pages = 1 ID Sheets and 3 Ans. Sheets)
(For Various Boards, Number of Subject + ID Sheet = Number of sheets)
9.2 Serial Nos. – must be the same in all sheets
9.3 PRC Seal – must be intact
9.4 No marking or dirt (once the answer sheets are already used, replacement of ID/AS set is not advisable)
9.5 Refer to Floor Supervisor if there are discrepancies.
10. Let the examinees insert their brown envelope underneath the ID Sheet.

11. Remind the examinees to use BLACK BALLPEN in printing and PENCIL in shading.
12. Let the examinees fill out the front portion of the ID SHEET.(Capital Letters)
a. Application No. (refer to their NOTICE OF ADMISSION) ex. ( 2 1_ _ _ _ _ _)
b. LAST NAME, FIRST NAME, MIDDLE NAME.

13. Filling up back portion of ID SHEETS (Pls. refer on page 5)

14. After filling out the back of the ID SHEETS, let the examinees SHADE the corresponding number of their
APPLICATION NO. and the LETTERS OF THEIR NAME using PENCIL NO.

15. Let the examinees fill out the Seat Plan, guided by one Proctor. (While the examinees writing the Attestation on Non-
Disclosure Declaration).

16. Instruct examinees to detach the last page of the answer sheet.(In detaching it must be along with the
perforated line)
16.1 The rest of the ID/AS SHEET will be placed inside the brown envelope.
16.2 Then let the examinees write the subject in all capital letters
(Ex. GENERAL EDUCATION)

17. Have them place the detached Answer Sheet under the flap of the Brown envelope, then place it on top of the
arm chair.

18. To be placed inside the PLASTIC ENVELOPE are the following:


a. Notice of Admission d. extra sharpener, pencils and pens
b. Official Receipts.
c. Valuables like, wallets, money, and Calculator Cover.
19. Collect the PLASTIC ENVELOPES by row. Place it below at the blackboard. Place their snacks in front of their
respective seats.

20. At 7: 45 am have the examinees attend to their personal needs and remind them to be back at 7: 55 am.
(Remind them that they are not allowed to go out as soon as they have their questionnaires with them).
21. Ask the examinees to empty their pockets and keep all other unnecessary things inside the transparent
envelope. (Gadgets in transparent envelope is strictly prohibited)

22. While waiting for the questionnaire, let one volunteer examinee lead a prayer.

RECEIVING THE TEST BOOKLET (TB) / TEST QUESTIONAIRES (TQ) FROM THE FLOOR SUPERVISOR:
23. Count the TB / TQ if it is enough for your examinees . (Ex. 24 examinees, TB should have 12 – A and12 – B)
24. Distribution of questionnaires is:
 SET A - Odd Seats Numbers (Ex. Seat # 1,3,5,7 and so on)
 SET B – Even Seats Number (Ex. Seat # 2,4,6,8 and so on)
24.1 Test Booklet / Questionnaires (TB / TQ) of the absent examinees will endorse to the Floor Supervisor
5 minutes after the distribution, write the:
 NAME OF THE EXAMINEE
 SEAT NUMBER
 ROOM NUMBER
25. Remind examinees to write their seat no. on the TB / TQ, let them check every pages of the TB /TQ if it is
readable and in complete pages, then let them shade A or B on their answer sheet.

WHEN THE EXAMINATION PROPERLY STARTS:


26. One Proctor will collect the brown envelope of the examinee, while collecting the brown envelopes check the
following:
a. Set of their Questionnaire (Ex. If the No. 1 receive SET A, If No. 2 receive SET B and so on)
b. Shading of A or B on their Answer Sheet.
c. Name of the Subject on their Answer Sheet.
26.1 Check if the examinees properly fill-up and shade their ID Sheets and return it to their respective brown
envelope.
26.2 After checking, all brown envelopes must be placed inside the Plastic envelope (ROOM WATCHER KIT
ENVELOPE).

27. While the exam is in progress accomplish the forms:
27.1 Room Watcher’s Report (2 copies)
27.2 Examinees Record of Attendance Form (2 copies) (Refer to the master list, write their names in
a Capital Letters)

28. Receive the brown envelope from the floor supervisor for keeping the ANS. Sheets for the designated subject.

SUBMISSION OF TEST BOOKLET / QUESTIONAIRES (TB / TQ) AND ANSWER SHEET:


 For Normal situation of examination this will be the instructions = If the examinees are done, instruct them to go outside
and avoid loitering in the lobby. They are not allowed inside the classroom as long as there are still examinees who are
answering. They may back 30 minutes before the next subject starts
 For State Public Health Emergency = After the examinee passed his/her TB/TQ and Ans. Sheet, instruct them to go back in
his / her seats. Shall be allowed to eat in batches (30 mins each) during breaks while seated in their assigned seats. Odd seat
numbers will eat first, followed by even seat numbers. While the first batch of examinees has their snacks/lunch, the other
batch can go to the restroom to attend their personal needs.

29. Upon submission of Test Booklet / Questionnaire and Answer Sheet, have examinees sign on the Examinees
Record of Attendance, indicate the time of submission, SET A or SET B and no. of pages of questionnaire.

30. Place the Answer Sheet inside the brown envelope provided by the Floor Supervisor. Place beside the used TB /
TQ of the examinee. (Proctors are not allowed to read the TB/TQ nor secure a copy or picture on it).
31. Proctor who receives the used TB/TQ and Answer sheet of the examinee will sign the Notice of Admission and
return the signed Notice of Admission to the examinee. (NOTE: Do not sign on the Notice of Admission while
they are still taking the examination, this is a wrong practice.)

ALLOTED TIME IS OVER OR ALL EXAMINEES ARE DONE:


32. Submit the brown envelope with exact no. of answer sheets and the used TEST BOOKLETS / QUESTIONAIRES to
the Floor Supervisor. (Do not wait for your Floor Supervisor to collect it from your room).

33. Upon submission on the first subject (Ex. GENERAL EDUCATION), both proctors will endorse the answer sheets
and the used test booklets to the floor supervisor and be back 30 minutes before the start of the 2 ndsubject, (Ex.
PROFESSIONAL EDUCATION).

PREPARATION FOR THE SECOND SUBJECT:


34. 30 minutes before the next subject, recheck again the examinees if they are seated at their assigned
seat/chair.

35. Distribute the brown envelope of the examinees with ID/AS Sheet inside. (Instruct the examinees to check if they
receive their own brown envelope and the ID/AS Sheet inside on it.)

36. Do again the STEP # 16, STEP # 17, STEP # 18 and STEP # 19.
37. Follow again the STEP of:
a. RECEIVING THE TEST BOOKLET (TB) FROM THE FLOOR SUPERVISOR
b. WHEN THE EXAMINATION PROPERLY STARTS
c. SUBMISSION OF TB / TQ AND ANSWER SHEET
d. ALLOTED TIME IS OVER OR ALL EXAMINEES ARE DONE

 For Examinations that have beyond 2 subjects, please follow the steps (PREPARATION FOR THE SECOND
SUBJECT, which is just same cycle of instructions).

SUBMISSION OF THE ID SHEET :( this may prepare on the last subject)


38. The Floor Supervisor will provide another brown envelope intended for the ID sheet of the examinees.
39. Collect the brown envelope of the examinees (only the ID Sheet inside on it).
40. Detach the ID SHEETS of the examinees (follow the perforated line).
41. Put the detached ID Sheets on the brown envelope provided from your Floor Supervisor.
42. Endorse the brown envelope to the Floor Supervisor with complete number of ID Sheets inside on it.
43. Wait until the examinees are done answering, then follow the steps:
a. SUBMISSION OF TB / TQ AND ANSWER SHEET
b. ALLOTED TIME IS OVER OR ALL EXAMINEES ARE DONE
44. Remind the examinee to send their ANNXE C – Post Survey to our designated email.

REMINDERS:
 Do not sleep and going out while the examination is in progress.
 Only Secondary – Math allowed to use BASIC CALCULATOR during Major Subject Only ( MATHEMATICS)
 Avoid talking with your co-watchers.
 Do not use the cellular phone or any gadget while the exam is going on.
 All used and unused test booklets must be accounted for, room watchers will be held liable for the missing test booklets.
 Avoid familiarity with the examinees. Do not copy names, addresses of examinees for any purpose.
 Do not give conflicting instructions to the examinees. Refer to the Floor Supervisor.
 Do not tolerate examinees talking with each other while the examination is in progress.
 If an examinee is caught cheating, do the following:
 Confiscate the material
 Let the examinee sign his/her name, write the name of the examination, date, subject, school, building, room and
seat number
 Allow examinee to continue
 Inform the Floor Supervisor and make a written report and indicate it in room watcher report.
 Do not make interpretations of the test questions.
 Do not rush the examinees.
 Answering the test questions to test you or make copies of the test questions is absolutely prohibited.
 Be Vigilant and Be Respectful.
 All examinees shall complete the ANNEX A , ANNEX B and RT PCR TEST or CERTIFICATE OF QUARANTINE
 Observe physical distancing of two meters all the time.
 Wear facemask (at least 3-ply surgical mask, preferably N95 and face shield).
 Bring 70% ethyl alcohol for hand disinfection.
 Room watchers/proctors are not allowed to eat nor remove their masks during breaks. They will be allowed to eat while the examination
is in progress inside the examination room.
 Windows will be opened to improve ventilation during breaks.
 Do not let your examinee talk to each other or rooming around during breaks.
 Bring your own ball pens, pencils and erasers to avoid borrowing from others.

Using BLACK BALLPEN, fill in the data needed in the boxes. For items 1—15, PRINT the data in CAPITAL LETTERS:

1. IN THE BOX LABELED BOARD OF, PRINT THE NAME OF THE BOARD.
Ex: PROFESSIONAL TEACHERS

2. IN THE BOX LABELED DATE OF EXAM, PRINT THE MONTH, DAY AND YEAR IN FULL. EX. MARCH 29, 2021 / MARCH 29,
30 & 31, 2021.

3. IN THE BOX LABELED SCHOOL GRADUATED FROM, PRINT THE COMPLETE NAME OF YOUR SCHOOL WHERE YOU
GRADUATED FROM YOUR BACHELOR’S DEGREE COURSE.(for units’ earner, write the name of your school where you
graduated your degree, not the school where you earned the units).

4. IN THE BOX LABELED LOCATION, PRINT THE LOCATION OF YOUR SCHOOL GRADUATED FROM. PRINT THE NAME OF THE
CITY OR TOWN & PROVINCE ONLY. Ex: CEBU CITY or TUBIGON, BOHOL.

5. IN THE BOX LABELED DATE OF GRADUATION, PRINT THE MONTH IN FULL AND COMPLETE YEAR (YYYY) OF YOUR DATE
OF GRADUATION IN YOUR BACHELOR’S DEGREE COURSE. Ex: MARCH 2014

6. IN THE BOX LABELED REVIEW CENTER ATTENDED, PRINT THE NAME OF THE REVIEW CENTER OR THE SCHOOL THAT
CONDUCTED THE REVIEW COURSE. PRINT SELF-REVIEW FOR THOSE NOT ATTENDING THE REVIEW COURSE.

7. IN THE BOX LABELED LOCATION, PRINT THE LOCATION OF THE REVIEW CENTER OR SCHOOL, PRINT THE NAME OF CITY
OR TOWN & PROVINCE ONLY. Ex: CEBU CITY or ARGAO, CEBU. FOR SELF-REVIEW LEAVE IT BLANK.

8. IN THE BOX LABELED DATE OF BIRTH, PRINT THE MONTH IN FULL, THE DAY (DD) (IF YOU ARE BORN BETWEEN DAY 1 TO
DAY 9, ADD 0 BEFORE THE NUMBER) AND THE COMPLETE YEAR (YYYY) OF YOUR DATE OF BIRTH. Ex: SEPTEMBER 08,
1977

9. IN THE BOX LABELED SEX, PRINT YOUR GENDER. Ex: MALE or FEMALE.

10. IN THE BOX LABELED TYPE OF EXAM, PUT A CHECK MARK ( / ) IN THE BOX COMPLETE . FOR CONDITIONED EXAMINEES
PLEASE PUT A CHECK MARK ( / ) IN THE BOX CONDITIONED then write the subjects below that needs to be taken.

11. IN THE BOX LABELED PLACE OF EXAMINATION, PRINT THE NAME OF SCHOOL/CAMPUS & THE BLDG. AND CEBU CITY.
Ex: UC MAIN, CEBU CITY

12. IN THE BOX LABELED LAST EXAMINATION TAKEN, PRINT THE MONTH IN FULL AND COMPLETE YEAR (YYYY) OF YOUR
LAST EXAM TAKEN. IF THIS IS YOUR FIRST TIME, JUST LEAVE IT BLANK.

13. IN THE BOX LABELED PERMANENT MAILING ADDRESS, PRINT YOUR COMPLETE HOME ADDRESS OR MAILING
ADDRESS.

14. IN THE BOX LABELED SIGNATURE OVER PRINTED NAME, PRINT FIRST YOUR COMPLETE NAME. Ex: JUAN G.DELA CRUZ
JR. THEN SIGN OVER PRINTED NAME.

15. IN THE FIRST PARAGRAPH OF THE ATTESTATION ON NON-DISCLOSURE DECLARATION, PRINT THE NAME OF THE
BOARD ON THE BLANK LINE PROVIDED. Ex: PROFESSIONAL TEACHERS / PHYSICIANS / RADIOLOGIC TECHNOLOGISTS/
OR OTHER NAME OF LICENSURE EXAM.

16. IN THE LOWER PORTION LABELED SIGNATURE OF EXAMINEE (Affiant), INDICATE YOUR SIGNATURE ONLY.

17. IN THE LOWER PORTION LABELED DATE, PRINT IN FULL THE MONTH , DAY AND THE YEAR March 29, 2021 (ON THE
DATE OF THE FIRST DAY OF THE EXAMINATION)

18. IN YOUR USUAL HANDWRITING, COPY THE FIRST PARAGRAPH ON THE SPACE PROVIDED STARTING ON THE FIRST LINE
WITH AN ARROW. THEN LEAVE ONE SPACE VACANT AND COPY THE SECOND PARAGRAPH.
NOTE:
 Instruct the examinees to shade the front portion of there is ID sheet once they are finish writing the ATTESTATION ON
NON – DISCLOSURE DECLARATION
 While the examinees are writing, accomplish the seat plan.
 Proceed to the STEP NUMBER 15.

PRAYER BEFORE THE EXAM


Dear Lord,
As I take the exam, I thank you that my value is not based
on my performance, but on your great love for me.
Come into my heart so that we can walk through this time
together. Help me, not only with this test, but the many tests
of life that are sure to come my way.
As I take this exam, bring back to my mind everything I
studied and be gracious with what I have overlooked.
Help me to remain focused and calm, confident in the facts
and in my ability, and firm in the knowledge that no matter
what happens today you are there with me.
