I, KAYOOM, S/O ABDUL KADER aged about 36Years, presently residing at Kodathi village,
carmelaram post, sarjapura main road Bangalore 560035 do hereby solemnly affirm & state on
oath as follows: -
I hereby declare that I’m residing at the above address since 20Years.
I declare that myself, my entire family members belong to MUSLIM FAMILY
which is Designated as (ಆದಿಕರ್ನಾಟಕ).

That this affidavit is made to produce before the concerned Authority to obtain
CAST CERTIFICATE AND INCOME CERTIFICATE for Self & family
I declare that my annual income is Rs. 20,000/- for all my sources. (I’m working
as a Dry Waste Picking)
For the Study purpose for my Son & Daughter, and They Name is MOHAMMAD
and TAYABA AKTAR, I need income & Caste Certificate.
I do hereby declare & verify that what all state above are true & that this my name,
signature & contents of this affidavit is true to the best of Knowledge and belief
information.
