FUNDAMENTALS IN NURSING

RETURN DEMONSTRATION - BED SHAMPOO

Anemor: Good day BSN-1B, we are group 4 and today we will show you the procedure on
bed shampoo. Bed shampoo is the washing of the hair and scalp of the patient in bed. It
is done to stimulate the blood circulation to the scalp, to increase client's comfort, and to
assess hair or scalp problems of the patient.

Preparation on Materials: Anemor


-Verify chart for a written order
Prior to the procedure, we need to verify the chart for a written order. It is important to
determine whether there is a doctor’s order about the type of shampoo to be used and
the best time of day for the procedure. If there is no type of shampoo medicated in the
patient's record, we will use the regular shampoo that the patient uses.
-Wash hands
Next, we have to wash our hands before having contact with our patients in order to
reduce the number of microorganisms in our hands and reduce the risk of transmitting
disease-causing microorganisms to our patients
-Introduce self
So I am done washing my hands, so let us proceed to the next step which is to introduce
myself to the patient. Remember, before entering the patient's room we need to knock.
“Ma’am I am Anemor, your student nurse for today. How are you feeling right now? This
morning mam we will do hair shampoo while you're in your bed, Is it okay with you
ma’am? Is it okay if we perform it right now? Or do you prefer doing it at a later part of
the day? “

This eases the anxiety of the patient and increases the patient's cooperation.
-Prepare equipments/materials needed and carry equipment at the bedside
Based on the patient's answer, I can now proceed to the next step which is to prepare
the equipment/materials needed.
1.Prepare the shampoo solution.
Since there was no written order from the patient's chart, I will use the shampoo
that the patient provided.
2.Fill pitcher with water
We have to fill the pitchers with warm water
3. carry equipment at the bedside and arrange them conveniently.
This is for easy access and so that we can avoid accidents and spillage.

The whole preparation of equipment/materials will help us save time and energy from going
back and forth to getting materials. Now let us have Anthony continue to explain the next steps
of the procedure.

Preparation of Patient (1-3): Anthony

Thank you Anemor for showing us the preparation of materials for the bed shampoo procedure.
After preparing and gathering the equipment needed for the procedure, we must now prepare
the patient. Preparation of the patient is critical to the success of the procedure because this will
help us create an environment where the patient has privacy, comfort, safety, and is in the
optimal position for the procedure we are about to perform.
1. Explain to the patient what you are going to do.
- To start the preparation of the patient, we need to explain to the patient what we are
going to do. We need to let them know the procedures we are going to take, so the
patient does not feel lost throughout the procedure. As nurses we are responsible for
ensuring that patients are able to understand their health, illnesses, medications, and
treatments to the best of their ability. When the patient knows what we are about to do
and why, he/she will be less anxious with the procedure thus, we can assure that the
patient will be more comfortable.
- “Good day maam/sir, I am nurse Anthony, and I will be taking care of you today. Today, I
will be shampooing your hair so that I can make sure that it is clean. By performing bed
shampoo, I will be able to promote your hygiene as well as stimulate blood circulation to
your scalp. Before we proceed, may I know if you have any questions regarding the
procedure?”
2. Close windows and screen bed
- After explaining to the patient the procedure, we should now close the windows and
screen bed. We do this because we want to protect the patient’s privacy throughout the
procedure.
3. Move bedside table away from head of bed
- After ensuring the client’s privacy, we must now move the bedside table away from the
head of the bed. We do this because we will be working in that area and making sure
that there are no obstructions, helps us provide nursing care safely and effectively.
Now let us have Dazell to discuss the next steps in preparing the patient for bed shampoo.

Preparation of Patient (4-7): Dazell


Thank you Anthony for showing us the steps 1 to 3. I am Dazell Ann Varron and I am assigned
to perform bed shampoo in steps 4 to 7. Now that we have already moved the bedside table
away from the bed let’s now remove the pillows and that is for step 4.

- Removing the pillows makes the patient lie flatly on the bed and helps to drain off the
water easily and keeps the shoulders dry.
- “Good Day Sir! I am your nurse for today and I will be preparing to wash your hair. Is it
okay for you if I remove the pillows so that you can lie down flatly on the bed?”
Step 5: Have the patient lie diagonally across the bed with the head near the edge of the bed
- A supine position facilitates drainage away from the patient’s face, eyes, and head and
helps the nurse to be able to work comfortably. Ask the patient to let you know if they
feel uncomfortable in this position at any time.
- “Okay sir can you lie down diagonally and put your head near edge of the bed. If ever
you are not comfortable in this position sir please let me know”
Step 6: Place a bath towel or folded blanket under the patient’s neck
- The towel on the patient’s neck helps in hyperextending the neck allowing the nurse to
have easy access to wash the patient’s hair.
- “Okay sir can you lift your head a little so that I can put this folded blanket under your
neck so that I can wash your hair without difficulty?”
Step 7: Place a bath towel under the patient’s head
- The cloth acts like a padding which helps support the muscles and prevents undue
strain and discomfort to the patient.
- “Okay sir can you lift your head again so that i can put this towel under your head to
prevent you from feeling uncomfortable”
That is all for steps 4-7. I will now hand you to Maque to continue in performing the next steps in
preparing bed shampoo to the patient.

Preparation of the Patient (8-10): Maque


Good Morning! I am Student Nurse Maque Antiga and I’ll be showing to you the next steps in
preparing the patient for bed shampoo.
8. Make rubber sheet into an improvised Kelly pad and place it under patient’s head
: Substitution for a rubber sheet. I'll be using a rolled towel and a black garbage bag as an
improvised Kelly pad. In doing this, this will prevent wetting the linens.

"Procedure in making the Kelly pad; Ma'am could you lift your head a little
so i can place this kelly pad underneath. this will prevent water from spilling ma'am"

9. Place a pail on a chair lined with paper and adjust through in place.
: In placing the pail make sure that the tip of the garbage bag is snip and the hole is totally
inside the reservoir to avoid spillage

10. Comb Hair and Massage the Scalp


: in doing this step make sure the patient is comfortable and not tense. This step is the
preparation step for the proper shampooing procedures. This will assess the degree of tangling
and status of the scalp and increase circulation to the scalp

"before contact with the patient the nurse will put on clean gloves to prevent contamination and
infection. okay ma'am i will comb your hair now to detangle your hair and i will massage it to
increase
circulation in your head."

Preparation of Patient (11-13): Ysabelle


Good day! I am Ysabelle Vidal and I’ll be showing to you the next step after combing and
massaging the patient’s hair. First is:
11. Protect ears and eyes:
After the patient’s hair is combed, I will use cotton balls to the ears and disposable washcloths
to the eyes to protect them from water and shampoo.

12. Protect water slowly and carefully on scalp and hair:


Using a disposable jug, take water from the disposable bowl and wet the patient’s hair. Check
that the patient is happy with the temperature of the water, then start at the front hairline and
allow the water to drain down and away from the face, avoiding the eyes and ears. Do not hurry
and be gentle. It is to promote patient comfort while washing their hair.

13. Pour the shampoo on scalp:


Put a small amount of shampoo on the palm of your hand and rub it into the patient’s hair and
scalp with circular movements.

The following stage after shampooing the patient is rinsing the hair, which will be discussed by
Jeimie.

Preparation of Patient (14-16): Jeimie


Thank you for showing us the steps 11-13 Ysabelle. Now I’ll be showing you the next step after
pouring shampoo on the scalp.
14. Rinse hair with warm water and repeat as often as necessary.
- After pouring a solution or shampoo on the patient's scalp, we now rinse it with warm
water in order to prevent irritation of the scalp from shampoo residue.
- "So ma'am, I'm going to pour warm water to your scalp to wash out the shampoo."
15. Do final rinsing with cold water.
- After rinsing the hair with warm water, we now rinse it with cold water to ensure that the
scalp remains clean for a long period of time as the cold water closes the pores, which
makes it less vulnerable to grease, oil, and dirt.
- "Now, I'm going to pour your scalp with cold water so that your hair will remain clean for
a long time."
16. Squeeze out excess water from hair and remove earplugs.
- After rinsing the patient's hair with warm and cold water, we now remove the earplug
after squeezing out the excess water from the hair which helps in drying by removing
excess moisture.
- "Okay, so I'm going to squeeze your hair gently to remove the excess water."
I will now hand you over to Angel to discuss the next steps in preparing the patient for bed
shampoo.

Preparation of Patient (17-21): Angel


Thank you Jeimie for showing us steps 14 and 16. Now, I will be showing the rest of the steps in
bed shampoo.
17. Pull rubber sheet, fold carefully and place on tray. Remove cotton balls and wash cloth
- Right after squeezing out excess water from hair and removing the earplugs let us now
pull out the rubber sheet, remember to fold it carefully and place it on tray. Placing used
materials to their designated places keeps us from using the same towel and also
through this, we can make sure that we are using clean equipments to the client.
Okay maam, now that I’m done rinsing your hair, I will now be removing the kelly pad slowly.
Please lift your head. Ok thank you maam.
18. Rub head with towel until hair is partly dry.
- Slowly rub the towel to the hair. Rubbing the head with a towel keeps the client from
getting chilled and also gives them a refreshing feeling.
Maam, I will now rub the towel to your hair to dry it. I’ll just do this gently. Maam, I'm done
rubbing your hair with the towel. So this time I’ll be replacing the pillow for to be more
comfortable.
19. Replace pillow with protected dry towel.
- When drying of hair is already done, we should replace the pillow. Take note that the
pillow should be protected with dry towel. This gives the patient a refreshing feeling and
also makes them comfortable.
This pillow maam is new and clean. Please lift your head. Alright, thank you.

20. Comb hair


- Aside from combing the hair, arrange the hair according to the patient’s preference. This
boosts the client’s self esteem.
Maam, I’ll now be combing your hair. Please tell me if the way I comb is painful.
21. Straighten bedding and make patient comfortable.
- Now that the procedure is almost done, don’t forget to straighten beddings, and make
sure that the patient feels comfortable after the procedure.
Alright, so we’re almost done maam. Before I go out allow me to put you back in your original
position.
I will now hand you over to Jian for the after care steps.

After Care (22-2): Jian


Thank you angel! Good Morning, I am Jian and I will be showing you on how to do the aftercare
of the bed shampoo
22. . Tidy the unit
1. Bring equipment to the utility room. Clean and return to proper places.
- After straightening the bedding, tidy the unit and bring the equipment to the utility room
and return them to their proper places. It is important to do this to ensure the patient’s
safety.
“Ma’am I am done shampooing your hair but I will return first the equipment to the utility room
and come back later for the charting”

2. Wash and rinse patient’s comb and return to bedside,


- Next before returning the comb to the bedside wash the patient’s comb in the sink. This
is needed to reduce the risk of infection that might affect the patient’s condition.
“Ma’am I will wash first your comb before returning it to your bedside”

After shampooing the patient and returning the equipment that was used proceed to charting
and this will be shown by John Riel
“Ma’am I am now going to do the charting”
Remove gloves, then wash hands.

returning all the equipment, it's time to document the time were she was given a shampoo and
her reaction or feelings during shampooing.

2. Reaction of the patient

shampooing the patient, she feels comfortable and safe. And I don't see any problem
with the patient.

Thank you and once again we are from group 4 and we have just performed bed shampoo
procedure.
