How to Create a Google Form and QR Code

1. Open Google. Click on the nine dots. Scroll down and click “Forms” at the bottom.
Make sure you use your own g-mail, not the MOE gmail.

2. When “Forms” is open, click on Blank.

3. Click on “Untitled Form” and write down the title of your form. Write down the description also,
e.g. data, time, venue, etc.
4. Click “Untitled Question” and write down your first question, e.g. Name. Then click on the arrow
of “multiple choice”.
5. Since respondents need to write down their name, so click “Short answer”.

6. The following will appear. Click on the circle at “Required” so that the respondent must answer
this item.
7. To create the second question, click “+”:

8. Then, write down your second question and choose “Short answer” if your question is not a
multiple choice question.
9. Repeat the same procedure until you have finish typing all the questions. Then click “Send”.

10. By putting the cursor at any question in the form, right click:

11. Click “Create QR Code for this page”:


12. Download the QR Code.
13. Open the downloaded QR Code and “Copy”.

14. Open a blank Microsoft Word, and write the title, etc. Then paste the QR Code in this Microsoft
Word.
15. When the respondents scan the QR Code, your Google Form will appear, and they can fill out
the details and submit.
16. You can see the responses of the respondents in your original Google Form.

17. Get the Excel file by clicking “Link to Sheet”.
