INTRODUCTION

The Factory Acceptance Test (FAT) is a major project milestone where all materials, equipment,
appurtenances, systems and technology (hereinafter referred to as equipment) directly purchased by
MANILAWATER or by MANILAWATER-accredited suppliers or vendors are tested to ensure compliance
to the defined "FAT required items" in the project contract documents. Generally, a substantial financial

payment to the supplier/vendor is triggered by a successful FAT, therefore the FAT must be conducted
formally andbewitnessed bythe client/owner orhis/herrepresentatives and/or Project Manager, with a
formal record of discrepancies or non-conformities and how they are to be handled.

EXCEPTIONS, DEVIATIONS AND CONFLICT

Where the best interests of Manila Water Company, Inc. (MWCI) are served, Corporate Project
Management (CPM) group may deviate from this procedure. Any such deviation shall be agreed in
advanceandauthorizedinwritingbytheCPM-QADHeadwithconcurrence oftheCPMGroup Director.

Any agreed deviation shall be for a single use and repeat deviation shall require the review of this
documentation for necessary updating/revision otherthan the set periodicreview ofall CPMdocumented

OBJECTIVES AND SCOPE

Factory Acceptance Tests (FATs) are conducted:


. to verify that the equipment complies with the contract specifications.
. to ensure, thru actual tests, that the equipment performs according to its design and performance
standards.

. to ensure that any deviation from specifications or performance is identified and appropriate action
is taken before the equipment is shipped from the factory.
. to avoid project delays and/or contractual disputes arising from non-compliant or improper
equipment being delivered to the site.

. to ensure compliance with the specifications prior to installation.
. to ensure compliance with performance requirements, whether tested for FATor not.
. to avoid project delays, close-out problems, and/orcontractual disputesarisingfrom non-compliant

Thisguide aims to provide Manila Water and its subsidiaries the appropriate criteria to determine when
or whether a FAT is necessary. This also provides standards and procedures for the conduct and

DEFINITION OF TERMS

Factory Acceptance Test (FAT) After the completion of the manufacturing stage, FAT is usually
conducted at the vendor's plant prior to shipment and delivery to the
clientto ensurecompliancewithrespectto designandperformancedata
as defined in the relevant contract documents. The test may involve

physical and chemical tests, mechanical tests, performance tests, or a
combinationofthese. FATis conductedwiththe presenceofthe client's
representative/s.

Inspection Test Request and A controlled document used for requesting and documenting the
Report (ITRR) inspection.

Site Delivery Inspection Upon delivery to the site, and prior to installation, the equipment is
inspected to verify compliance with specifications.

Site Acceptance Tests (SAT) These are tests conducted on the installed equipment to validate the

results ofthe FAT. It servesto confirm that the equipment comply with
the requirements of the contract.

POLICIES

1. Criteria for conducting/attending the FAT

Thefollowingcriteria shall be usedto determine the requirement and attendanceto FAT:

b) Complexity of the equipment / system / technology
c) As a general rule, the following equipment shall undergo FAT:
. all valves and appurtenances sourced outside the Philippines that cannot be accommodated

. all pumps, blowers, and other rotating equipment;
. all generator sets, including Manual Transfer Switches (MTS), Automatic Transfer Switches
(ATS), synchronizing equipment;
. all electrical equipment (switchgear; transformer, Variable Frequency Drive (VFD),
softstarter, capacitor bank, etc. );
. new product brands/technology;

. existingaccreditedandpreferred brandwithnewordifferentmodelordesign;
. other critical equipment/system that need to undergo performance and or~precision

d) Equipmentclassifiedas "highlycritical" whereinnoneor incomplete testfacilitiesofthe full

3. Criteria for Waiving the FAT

MANILA WATER may decide to waive the requirement for FAT or opt not to send representatives to
thefactory testing activities. Inthiscase,QADHead, subject tofinalapproval ofCPMGroup Director
and/or Corporate Operations Group (COG) Director, as appropriate, may recommend waiver of FAT
requirement for any of the following:

. the project consists of replacement of existing equipment with another of the same brand and
model number;
. small or off-the-shelf equipment, materials or technology, as confirmed by the
supplier/manufacturer;

. equipment ofthesamebrandandspecifications whichhavejust undergone FAT(oneyearorless);
. equipment which are in common use in MANILA WATER facilities, and with no issues:
. special cases such as emergencies or projects with time constraints as recommended by the
Project Manager(PM)/Construction Manager(CM);
. equipment classifiedas "Not FATrequired" items asstipulated in the contract.

Waiverof FATdoesnotimply unconditionalacceptanceofthe equipment. ThePMshouldensurethat

the Contractor submits the relevant factory test results and Certificate of Inspection and Quality
Compliance (CIQC) from the manufacturer. LE
If Manila Water decides to waive or does not require FAT, the Contractor still has the option to
conduct/witness FATon his own. All costs for this activity shall be to the Contractor's account and
shall not be charged to Manila Water, and therefore should have no implication to the project time
and cost.

The Contractor/Supplier shall be responsible for facilitating all necessary travel documents, in caseof
foreign FAT. Travel and accommodation for MANILAWATERrepresentative/s shall beto the account
of MANILA WATER.

5. Criteria for conducting/attending SAT

SAT is conducted after the equipment is delivered to the site. It should mimic the tests that are

conducted in the FAT, and the results should mirror the same. SATis even more critical for those
items which have not undergone FAT.

As a rule, no equipment should be installed without first being inspected as part of the SAT. All
equipment should undergoSATas a pre-requisiteto project close-out.
