    RE : EMPLOYMENT CONTRACT

    Dear Mr. (Name),

    We are hiring you as Position effective DATE under the following terms and conditions:

    1. Your employment will be on a CONTRACTUAL basis for Three (3) months or covering the
    period of (Date) up to (Date) 2013 which will automatically terminate on the latter date without
    need for advance notice.

    2. You are required to work five (5) days a week or to render overtime work (if needed). Unless
    otherwise adjusted/shortened by the Company in accordance with existing laws and business
    conditions.

    3. You are to receive a daily gross compensation of Four hundred Forty-six Pesos (P446.00).

    4. You are on a no work-no pay basis.

    5. Your wages are payable every week unless otherwise re-scheduled by the Company for just
    causes.

    6. Deductions shall be made in cases you are shown to be responsible for any loss or damage to
    company property, materials and equipment.

    7. Your contractual employment with the Company does not entitle you to vacation and / or sick
    leaves, nor to any other benefits that are or may hereafter be granted only to regular and
    permanent employees, except those which the Company as a matter of policy and upon its
    discretion, extends to all employees regardless of status and those provided by law.

    8. You are not entitled to any separation pay upon expiration of contract, termination of
    employment, or resignation, except those, which the Company as a matter of policy and upon
    its discretion, extends to all employees regardless of status, and those provided by law.

    9. The Company reserves the right to terminate your employment even prior to the expiration of
    your contractual period, for any of the just and authorized causes provided by existing law and
    for your having failed to satisfactorily meet and comply with Company standards, conditions
    and requirements.

    10. You agree to abide by all the Company Rules and Regulations, which shall be your duty to
    study and understand.

    11. It is agreed and understood that in cases of disputes which may arise between parties herein
    with respect to the interpretation and enforcement of Company personnel policies or matters
    concerning wages, hours of work and other conditions of employment, the same should be
    initially resolved through the Company’s grievance machinery.

    12. You agree that all records and documents of the Company and all information or secret
    processes pertaining to its business or affairs are confidential and unauthorized disclosure or
    reproduction of the same will not be made by you anytime during or after your employment.
    You agree that any breach of confidentiality will constitute sufficient ground for immediate
    termination of your employment for cause, without prejudice to any liability for civil damages.

    If you are agreeable to all of the conditions stipulated above, your signature on the space provided will
    signify your conformity and acceptance of the foregoing terms and conditions.
