RE: STERN WARNING LETTER

This is a written stern warning being issued to you for an unexcused absence, which
is a violation of company policy.

Specifically on the following date below, you did not come to work as assigned. In
addition, you failed to notify your Trainer that you would not be present prior to the
start of your shift.

23-Mar-10 11.30 am - 9.00 pm AWOL
29-Mar-10 9.00 am - 6.30 pm AWOL

It is important to understand that you are to be at your assigned place of work prior
to the beginning of your shift. In the event you are not able to be present before the
start of your shift, you are to notify your Trainer prior to your start time and request
permission to be absent. Although your Trainer may try to accommodate your
request, understand that they are not compelled to do so.

You are advice in your own interest not to repeat the above conducts again. Should
similar incident recur in future, stern disciplinary action will be taken against you,
which may include termination of your contract of service without compensation.

You are to acknowledge receipt of this letter by signing on the duplicate of this letter
and returning it to us.
