ADMISSION IN GENERAL WARD

PURPOSE

1. To ensure the patient and relatives a courteous welcome into the hospital
2. To orient the patient and family to the hospital set-up
3. To provide immediate care to patients upon arrival at the Nursing Units

PROCEDURAL FLOW

1. Receives call from Admission Office or Emergency Room


1.1 Receives preliminary telephone endorsement of patient’s
1.1.1 Name & Room Number
1.1.2 Conditions & Amenities needed
1.1.3 Chief Complaints
1.1.4 Procedure Done
1.1.5 Attending Doctors
2. Prepares room for Admission
2.1 Rechecks room for readiness. Turns on the air conditioning, light, TV, refrigerator,
etc.

3. Receives patient from Admission or ER


3.1 Welcomes patient with a smile and introduce self.
3.2 Takes patient’s weight and height and ask for allergies. (If not done in ER)

Note: Take into consideration the manner that the patient is endorsed. For
stretcher-borne patient and cannot stand, weight-taking is exempted While the
height shall be measured through a caliper.

3.3 Receives the following from the Admission/ER


3.3.1 Doctor’s Order Sheet
3.3.2 Consent properly signed
3.3.3 Patient Data Sheet
3.3.4 Patient’s Name Tag
3.3.5 Admission Kit
3.4 Checks on the ID wristband of patient & the completeness of the Admission Kit.
3.5 Ushers patient and relatives to the room.

4. Renders preliminary Nursing Care


4.1 Provides privacy. Ask relatives to leave the room unless they will assist in
undressing the patient.
4.2 Helps the patient change clothes to patient’s gown and assist him into a comfortable
or required position in bed.
4.3Takes care of the patient’s clothing and valuables. Explains hospital policies on
safekeeping.
4.4 Orients patients and relatives to hospital policies and usage of room amenities
(Room Checklist)
4.5 Takes vital signs
4.6 Make an initial assessment of all pertinent observations and information gathered
from the patient and or family during the admission interview.
4.7 Carries out admitting doctor’s order.

5. Retrieves old chart form the Medical Records per Doctor’s order.

6. Documents nursing care rendered.
