Directions: Using the following matrix, organize your unit Assessment Plan. Use one matrix for each grade level. Number each lesson,
assessment and rubric so they are identified together. Design 1 Unit Assessment Plan (UAP) for three (3) consecutive classes for your content area
that incorporates:
a. Formative assessments (5 varieties of assessments) and a one (1) Summative assessment. (Total assessments for ULP, NOT per lesson)
b. Rubrics for each assessment (6 in total).
c. A mix of items within each assessment that assesses at least 3 of the following dimensions: Knowledge, Comprehension, Application, Analysis,
and Synthesis. (Use Matrix to indicate dimension)

On a separate pages attach


a. Sample assessment with instructions, questions (if applicable -for Science - Define photosynthesis and describe the process), and diagrams
(e.g. Venn, Charts, Tables); if assessment is not written, describe what you ask student to do and response they give.
b. Identify either Formative or Summative on matrix; and mark the dimensions assessed on matrix.
c. The rubrics for each assessment.
