SUBJECT : Investigation Report on Alleged Theft Incident Happened at SBC Edsa- Magallanes
Parking Area.

I. Reference:

4. Incident Report dated 29 November 2018 submitted by SIC Inocencio, Roderick,
assigned Perimeter Guard of SBC Magallanes –Edsa Branch.

II. Authority:

1. Verbal Instruction from Frederick Base


2. Inherent responsibility of Deputy OIC of BOS Security Solutions and Services Inc.

III. Matters to be Investigated:

1. To identify the person/ SG violated the Security Procedures.


2. To determine the circumstances that led to the incident ( Theft Incident)
3. To establish other information that will help to improve the implementation of Protocols
& Procedures.

IV. Facts of the Case:


1. On or about 0656H 28 November 2018, Mr. Rommel Baltazar and Ms. Beth Baltazar on
board with their service vehicle SUZUKI gray with plate# NYI 214 arrived at SBC
Magallanes- Edsa Branch and parked their car at parking area along EDSA near the
ATM machine.
2. At around 1900H same day, Mr. Rommel Baltazar arrived at the Branch.
3. At around 2035H same day, Mr Baltazar went inside the bank, accordingly, after
informing his wife, Mr. Baltazar went out to the bank and left his cell phone and wallet
inside his car and left the Branch.
4. On or about 2050H same day SG Pontaniel, Alex while conducting Roving Activity.
During that time he noticed that Mr. Baltazar was still awake setting at the passenger side
(front) while the door of his car was open( front/ passenger side). The said guard
continues roaming the perimeter.
5. On or about 2145H SG Pontaniel left his position (corner of the branch) and proceeded to
the entrance door of the branch in order to log the personnel (with car) who were leaving(
time out) the Branch.
6. At about 2200H same day, SG Pontaniel while roving the area he observed that the rear
door (other door) of the car (driver side) was open and he thought that Mr. Baltazar was
still awake and intentionally open the said door.
7. On 2209H same day, Mrs. Beth Baltazar, employee of the branch came out from the bank
and proceeded at the parking area. Upon reaching, she woke up his husband sleeping at
their car. When Mr. Balazar was awake, he noticed that his lunch bag containing his
wallet and cell phone was missing. Items missing inside his wallet; 4200 pesos, Drivers
License, SSS ID, Pag-ibig ID, and TIN ID.
8. On or about 2230H same day, SIC Inocencio, Roderick immediately informed the
Security Department about the incident.
9. On or about 1030H 29 November 2018, SO Ereño left BOS Office and proceeded to SBC
Magallanes-Edsa Branch and conducted investigation thereat.
10. At About 1230H 29 November 2018, SO Ereño, SO Masada, SIC Inocenco together with
the complainant Mr. Baltazar proceeded to the Barangay Office and Police Station of
Magallanes to execute formal complaint against the unknown culprit.

V. Findings:

1. Mr. Baltazar accidentally fell asleep while waiting for his wife. He forgot to close the
door of his car.
2. Before buying foodstuff he left his personal belonging placed at the rear set of the car to
avoid rob while outside the perimeter of the branch. However, when he came back he
forgot to put his wallet to his pocket until he fall sleep.
3. The perimeter of the branch has a blind spot from the line of sight of the guard since the
branch is situated at the corner of EDSA and Magallanes Road.
4. SG Pontaniel, continuously conducting roving inspection at the perimeter of the branch
however, on 2150H he proceeded to the main door of the branch in order to record at the
logbook the personnel(with car) that are leaving the Branch then after, he conducted
checking on the three (3) vehicle parked at the parking area along Magallanes road.
5. Sleeping at the car is a routinely activity of Mr. Baltazar while waiting his wife, based on
the observation of the guards assigned at the perimeter.

VI. Discussion:

Based on the facts gathered by the investigator, the owner of the lost belongings
demonstrated negligence in exercising his inherent responsibility in view of the fact that incident
was happened in his presence. Our guard was continuously conducting roving inspection at the
area, however, since the branch was situated at the corner of EDSA and Magallanes Road, part
of the perimeter consider as a blind spot on the line of site of our guard. Due to the given
situation, the culprit takes advantage while the guard was positioned at other side (along
Magallanes) of the perimeter.

VII. Conclusion:

With all indications, Mr. Baltazar was an alleged victim of Salisi Gang at SBC Edsa-
Magallanes Branch Parking Area.

VI. Recommendation:

1. Strengthen the security operation of the Bank Perimeter through:

a. Consistent information dissemination/ awareness Activity of premises guard to


remind the car owners (client/ non client and employees) that they need to secure
their personal belongings in order to avoid by being victim of salisi gang or other
organized group.
b. Increase the visibility of guards through continues conduct of roving.
c. Avoid habitual activity to avoid pattern (asing) of movement.
d. Positioning of perimeter guard at strategic point (corner).

2. Increase Support System

- Building Guard should support the perimeter guard.
