PERFORMANCE EVALUATION CHECKLIST FOR BABY BATH

PURPOSES
• To identify potential problems in the newborn, ensuring safety, and the prevention
of infection

ASSESSMENT

• Presence of pain and need for adjunctive measures before the bath

EQUIPMENT
•Cotton balls or disposable wash cloth
•Neutral soap
•70% alcohol
•Petrolatum gauze

DIRECTIONS:Mark each step of the procedure following the rubric below denoting
the skill performance of the student. Total the scores and look for the equivalent
grade on the transmutation table. The Remark section is for suggestions that will help
improve the
skills of students.

1. Secure consent. Wash hands. Check the temperature of the
water. If normal, prepare the necessary articles for bathing.
2. Close windows to prevent draft. Undress the baby. Observe for
any untoward signs in the body.
3.Use cotton balls or soft, disposable wash cloths to wipe eyes
face and outer ears.
a. Eye are wiped from inside to corner outward
b. Face and ears wiped using S-motion
3.Use a neutral soap-check pH.Clear water may be used if
infants skin is dry.
4. Carry the baby using “football” hold. Fold the external ears to
cover them and prevent water from entering. Wash infant’s head
using gentle circular motions.
5. Tilt head back to cleanse neck.
6. Hold the baby’s arm supporting the back and let the baby set
on the basin or tub. Bathe torso and extremities quickly.
7.Inspect umbilical cord. Check area for bleeding or foul odor. A
drying agent such as 70% alcohol may be applied several times
daily ( accourding to institutional policy). Do not cover with diaper.
Dressings are not used.
8. Cleanse genital area of male infants .
a. Cleanse penis without retracting foreskin
b. Circumcision care -keep area clean. Place sterile petrolatum
gauze over area for first 24 hours change after voiding. Observe
hourly for bleeding. Position infant and diaper to avoid friction.
9. Cleanse genital area of female infants
a. Wash vulva from front to back
b. Wipe vulva with cotton balls, using 1 stroke in front to back
direction.
10. Bathe buttocks using a gentle, patting motion. Keep area
clean and dry to prevent diaper rash. If rash does occur,
protective ointment (zinc oxide) may be used. Exposure of
buttocks to air or heat lamp is helpful.
11. Put on the bonnet, dress, mittens, diaper. Wrap the baby and
make him/her comfortable
