Self Mid-Term — Paige Papineau When working with patients, I ensure they're safety by utilizing a gait belt and/or parallel bars when performing
balance or gait activities where there is an increased risk for falls. When I need assistance with guarding or
advice of proper amounts of flotation devices in the pool, I am not afraid to ask for help. Over the past 6 weeks,
I have continued to improve my ability to progress/regress activities based on patient presentation that day. I
have gotten better at taking vitals during evaluations to ensure that a patient is safe to exercise. I am mostly
independent with simple patients, but require supervision more than 25% of the time with complex patients.

Self Final — Paige Papineau I ensure pt safety by utilizing a gait belt, parallel bars, AD, or second person when necessary. I often use
flotation devices to ensure pt safety while in the pool. I am able to recognize when it may not be safe for a
patient to exercise through their vitals and listening to their subjective complaints.

CI Mid-Term — Chris Hinze Paige consistently demonstrates good safety skills for patients and self during all patient encounters. Utilizes
safety equipment (ie gait belt) appropriately and consistently to maximize patient safety. Demonstrates

2.   Professional Practice – Professional Behavior


Self Mid-Term — Paige Papineau I arrive prepared, am timely everyday, and follow the dress code. I am receptive to positive and constructive
feedback from my CI and other staff, and maintain productive and professional relationships with staff and
patients. I protect patient privacy and modesty by following HIPAA and ensuring that curtains remain closed in
the locker room.

Self Final — Paige Papineau I continue to arrive prepared and on time every day. I have significantly improved my time management and
transitions between patients to ensure starting on time for each appointment. I continue to follow my clinical
site's dress code and demonstrate professional behaviors during all patient interactions.

CI Mid-Term — Chris Hinze Paige consistently demonstrates professionalism in interactions with patients and coworkers. Accepts and
absorbs constructive feedback very well. Small area for improvement includes punctuality/time management in
regards to pt. appointment times, especially when transitioning from one area of practice to another (ie pool to
gym) - although this has improved considerably over the past 2-3 weeks.

3.   Professional Practice – Accountability


Self Mid-Term — Paige Papineau I adhere to the policies and procedures set in place by the GT Pavilions. I maintain patient confidentiality by not
leaving patient documents unattended in areas where others may see them, by always locking my computer
when leaving it unattended. I am not afraid to acknowledge mistakes that I have made, areas of knowledge that
I am unfamiliar or less confident in, and when a treatment session did not go as well as it could/should. When I
make a mistake, need to edit documentation, or receive feedback for areas of improvement, I fix them in a
timely manner.

Self Final — Paige Papineau I continue to adhere to the policies and procedures at my clinical site and quickly acknowledge and fix my
mistakes. I am not afraid to arrive early, stay late, work through lunch in order to meet the patient's needs.
While I have gotten much better, I would like to continue to improve my ability to provide services that go
beyond the expected standards of practice to best help meet a patient's needs.

4.   Professional Practice – Communication


Self Mid-Term — Paige Papineau I am able to adjust my communication style to meet the needs of the patient. I have worked on communicating
with various patients varying in educational levels and cognitive levels. I have communicated with various
members of the interdisciplinary team for effective patient care. I demonstrate active listening though asking
clarification questions, verifying patient information through repeating back what they have explained to me,
etc. I recognize that my lack of confidence can be picked up in my verbal and nonverbal communication and
can affect the patient. I have been able to change verbal and nonverbal cues such as volume, tone to best
communicate with the patient. I need to improve at initiating communication in difficult situations. I continue to
require supervision greater than 25% of the time with complex patients.

techniques including verbal, visual, written, demonstration, etc. I am able to recognize when a patient is
fatigued or painful secondary to their nonverbal communication. When one form of communication is not
effective, I am easily able to transition to other forms in order to meet the patient's need.

communication effectively, and adapts communication style to meet the individuals needs without cues or

Self Mid-Term — Paige Papineau I have interacted with patients from a variety of backgrounds with a variety of varying beliefs, values, etc.
Whether I agree or not, I treat patients with the same amount of respect and provide care in a non-judgmental
manner. I recognize that I as well as patients have social and/or cultural biases. As the majority of my patient's
are elderly, it is important to recognize and respect generational differences in order to provide the most

6.   Professional Practice – Professional Development


Self Mid-Term — Paige Papineau Throughout the past 7 weeks, my CI and I have formal weekly meetings to review what went well the previous
week, discuss what I need to improve upon, and set goals for the upcoming week. I consistently ask for
guidance/assistance as needed. When we have free time, my CI and I will review different examination and
intervention techniques in order to improve my clinical skills. Additionally, we have had conversation related to
the practice of physical therapy including patient-PT professional relationships, setting + sticking to boundaries,
insurance, advocating for patients, etc. I will be providing an in-service for both my classmates and the GT
Pavilions staff by the end of my clinical rotation.

Self Final — Paige Papineau Throughout the last 7 weeks, my CI and I have continued to have regular check-ins to review the achievements
and to set goals. I ask for guidance/assistance in unfamiliar situations or to address my limitations, and am
always open to feedback from my CI, patients, or other staff. I have provided an in-service for my classmates
on aquatic therapy, and provided an in-service to the rehab team on burnout in the PT field.

CI Mid-Term — Chris Hinze Paige consistently seeks out feedback regarding her performance of procedural interventions and clinical
decision making from CI or other clinicians, as appropriate. Demonstrates good insight into strengths and
limitations and self-assessment is consistent with CI assessment.

CI Final — Chris Hinze Paige has continued to seek out feedback from CI during this clinical education experience. Paige has also
provided staff education through an in-service on "Burnout in PT", which the staff thought was interesting.
Hopefully Paige will continue to value ongoing professional development and mentorship once in clinical
practice by engaging in high quality continuing education programs and seeking employment that emphasizes a
strong mentorship program for new professionals.

7.   Patient Management – Clinical Reasoning


Self Mid-Term — Paige Papineau I am improving my ability to adjust my plan of action/ goals based on patient presentation. Over the past 7
weeks I have gotten much better at progressing/regressing interventions. I recognize that I do not know
everything and have limited experience, and know when to ask for help. I am not always confident in my
assessment or clinical reasoning when a patient does not meet the expectation, requiring my CI to assist. I am
mostly independent with simple patients, but require supervision and assist with complex patients. I continue to
require assistance in determining when its time to transition from pool to land and when I need to change
approaches.

Self Final — Paige Papineau Over the final seven weeks, I have significantly improved my clinical reasoning skills. I utilize information
from various sources including class notes, textbooks, conversations with my CI and co-workers, and
occasionally discussions with professors and classmates. I am able select/alter interventions based on patient
preference. For example, a patient has recently c/o of bruised ribs which were aggravated by prone positioning;
therefore, I modified the exercises to different positions that were not as aggravating. I recognize my limits as I
remain inexperienced compared to other members of the rehab team, and am not afraid to seek guidance or
assistance with things that are unfamiliar to ambiguous.

CI Mid-Term — Chris Hinze Makes simple clinical decisions independently. Requires minimal to moderate guidance for more complex
clinical decision making.

CI Final — Chris Hinze Paige has achieved entry level performance for this performance criterion. As Paige continues to acquire
clinical experience with a variety of patients, her ability to make sound clinical decisions with commensurately
sound rationale will continue to improve.

8.   Patient Management – Screening


Self Mid-Term — Paige Papineau When performing a chart review, I look at the reason for the referral, any notes from the doctor, the provided
pmhx, the intake form provided by the patient to determine an initial plan of action, any omhx that may effect
or need to be taken into account in the plan of care, and any indication that a patient may not be appropriate for
PT.

Self Final — Paige Papineau I continue to complete a comprehensive chart review while prepping for a new patient. I will often ask my CI to
review nursing notes from cottage residents. I have observed a screening in the inpatient setting to determine if
the patient required further PT evaluation.

CI Mid-Term — Chris Hinze Paige routinely reviews medical history from patient intake forms and through subjective examination. Student
can further grow in this domain by consistently reviewing EHR (when available) for ALF patients; and/or
screening for consultation with other disciplines (OT, SLP, etc).

CI Final — Chris Hinze Paige has demonstrated improvement in this area by more consistently reviewing the EMR prior to a patient
encounter and by coordinating with the MDT, when appropriate. Paige has also demonstrated improved ability
to screen for referral to other disciplines, eg to orthotist for consultation for unloading knee orthosis on a pt. w/
advanced knee OA.

9.   Patient Management – Examination


Self Mid-Term — Paige Papineau I am able to complete the subjective portion of the evaluation independently for most patients. From this
information along with information from my chart review, I am able to make predictions on how a patient will
perform throughout the session. I have improved my ability to complete the objective and prognosis
components of the evaluation. I am mostly independent for simple patients, but continue to require assist for
complex patients. Examination test and measures I have assessed include aerobic capacity, ergonomics/body
mechanics, gait and balance, MMT, ROM, pain, posture, reflexes and sensation.

Self Final — Paige Papineau Over the past seven weeks, I have improved my examination abilities. At this time, I am nearly independent
through the whole examination process. I have improved my efficiency and am now able to complete a
thorough examination and have time to provide a few minutes of treatment. Additionally, I regularly re-examine
patients during progress notes and interim sessions to track patient progress. Can continue to improve upon my
efficiency and prioritization of exam procedures.

CI Mid-Term — Chris Hinze Level of supervision for this criterion has progression from 100% to 0-25% during this interim. For simple
patients/conditions Paige requires very little guidance. Paige does require some consultation/direction for more
complex patient presentations. Areas for improvement are prioritization of tests/measures during an initial exam
(especially for more complex presentations), efficiency of performing necessary tests/measures, and
reexamining the pt. status as indicated w/o prompting from the CI.

CI Final — Chris Hinze Paige has achieved entry level status for this performance criterion. Paige independently performs a subjective
Hx review, selects appropriate tests and measures, and interprets findings w/ accuracy.

Self Mid-Term — Paige Papineau Over the past 7 weeks, I have progressed to being able to complete examinations mostly independent with
simple patients, requiring some assist with more complex patients. I have been able to complete subjective
questioning, strength testing, goniometry, sensation testing, assessing balance, gait patterns, and vitals. I am
able to make connections between the patients presentation and objective impairments to their activity and
participation limitations. I need to continue to improve my ability to make clinical judgements and my
efficiency in reaching clinical decisions.

Self Final — Paige Papineau Over the final 7 weeks, I have improved upon my evaluation skills. I am able to identify functional limitations,
impairments, and potential participation limitations based on the information gathered through my examination.
And I have improved upon my ability to make clinical judgements on interventions, changes in AD, etc. based
on the examination.

Self Mid-Term — Paige Papineau Over the past 7 weeks, I have improved my ability to differentiate between diagnoses to identify the primary
diagnosis. I have been able to to hypothesize how a patient's pmhx, co-morbidities, age, health behaviors may
impact interventions and have gained experience with identifying a patient's prognosis, determining a patient's
frequency to be seen, and plans for d/c. I continue to require some support in this area for more difficult,
complex patients.

Self Final — Paige Papineau I have continued to improve my ability to identify a patient's prognosis and diagnosis and have significantly
improved my ability to communicate this to a patient. I am independent for simple patients and require minimal
guidance for complex patients.

CI Mid-Term — Chris Hinze Paige establishes a clear diagnosis/prognosis for simple cases with confirmation from CI. For more complex
patient presentations student requires minimal-moderate guidance from CI. Areas for improvement include
being able to clearly and efficiently summarize this information and relay it to the patient to enhance their
understanding of their condition.

CI Final — Chris Hinze Paige has demonstrated excellent progress in this area from the start of this clinical to conclusion. Since
midterm, Paige has made great strides in synthesizing personal/social/environmental factors and how they
influence the pts prognosis. Further, Paige has gained great confidence in her ability to educate patients/families

examination findings. Student establishes and modifies POC as necessary for simple patients with confirmation

Self Mid-Term — Paige Papineau I ensure patient safety through utilization of a gait belt during gait or balance activities and ask my clinical
instructor for help as needed or if I am unsure of the patients level of assist. I have made significant
improvements in my ability to sequence, plan, and adapt interventions. I have made improvements in my ability
to describe exercises. I continue to require guidance for interventions for more complex patients. I often
consider prevention, health, and wellness when prescribing patients with their HEP. For example, I have created
impairment specific HEP in additional to general body HEP for interested patient's to not only prevent re-injury,
but to prevent injuries throughout their whole body and promote regular activity. I try to include self-
management tips and exercises within my POC to create autonomy and empower my patients. While I have
made great gains, I continue to need assistance and have lots of room for improvement.

Self Final — Paige Papineau Over the final 7 weeks of my rotation, I have continued to improve my ability to select and execute
interventions in a safe, effective, and fluid manner that are consistent with the POC. I have focused on
improving my ability to incorporate prevention, health, wellness, and fitness interventions and education to
improve a patients quality of life and overall health and wellness.

CI Mid-Term — Chris Hinze Student has demonstrated the ability to effectively and safely deliver manual therapy techniques (soft tissue
mobilization, joint mobilization), therapeutic exercise in both land and aquatic settings, patient/family
education, and functional training independently with consultation from CI. Student is demonstrating improving
quality and efficiency in delivering procedural interventions.

CI Final — Chris Hinze Paige effectively and efficiently delivers interventions with independence at this time. Paige has demonstrated

14.   Patient Management – Educational Interventions


Self Mid-Term — Paige Papineau I have educated patients through demonstrations, verbally, written handouts and can easily switch approaches
when I identify when I am not being effective with my current approach. Topics and tasks that I have educated
on include workplace ergonomics, activity cycling, use of RPE scale to determine progression/regression of
strength and aerobic training, pt diagnosis/ involved anatomy/ tissue healing, proper body mechanics during
exercise activities, and a large variety of exercises emphasizing many parts of the body. I continues to require
assistance over 25% of the time.

Self Final — Paige Papineau I have made great improvements in my ability to educate my patients. Over the last seven weeks I have
continued to improve my effectiveness in the previously mentioned topics at last interim, and have focused on
improving my ability to improve on more complex, whole body wellness topics including pain, sleep, and
nutrition. I have also created educational/helpful material for nursing staff regarding anterior and posterior hip
precautions.

CI Mid-Term — Chris Hinze Student demonstrates ability to provide basic patient education via demonstration or written formats, such as
home exercise program handouts, without guidance. Paige can further grow in this area by expanding the
impact their education to include education on ergonomics, lifestyle (ie diet/nutrition, sleep, stress management,
physical activity), home safety modification for fall prevention, etc.

Paige regularly provides pt education through various forms to help her pts meet their goals. Paige has also

Self Mid-Term — Paige Papineau I think documentation is one of my stronger areas. I am able to document accurately following the guidelines
and format required by the site. However, it does take increased time. I have been involved in documenting
evaluations, interventions, plan of care, d/c planning. Through my documentation, I am able to justify physical
therapy services and am able to document in an organized manner and am working on improving my efficiency.
I have significantly improved my efficiency with documenting evaluations, however; continue to work towards
being efficient with progress and d/c notes.

Self Final — Paige Papineau Documentation continues to be one of my stronger areas. I am able to document accurately and succinctly in the
format required by my site. I have greatly improved my efficiency in completing evaluations, progress
notes/recertification, and discharge notes. I have been involved in writing recertification notes for further
insurance authorization. I am over 90% independent in my documentation.

Self Mid-Term — Paige Papineau Have had the opportunity to complete the Berg Balance test, FGA, 5x STS, TUG, Gait speed, 2MWT, short
physical performance battery, Oswestry, etc. and have been working towards scoring and interpreting them
correctly. I have made significant improvements in completing these tests efficiently and effectively. I have
used current research to compare patient scores to age-related norms and determining the minimal clinically
important difference to help assess the patient's performance and set realistic and effective goals. I have
improved my ability to select outcome assessment measures that are appropriate for the individual. Still require
assistance/collaboration from CI over 25% of the time.

Self Final — Paige Papineau Over the last 7 weeks, I have improved my ability to select and interpret several outcome assessments and have
continued to improve my efficiency and effectiveness. I utilize current research to identify age-related norms

Self Mid-Term — Paige Papineau While I have not directly been involved in scheduling a patient, I commonly relay schedule changes to my CI,
review a patient's schedule with the individual, or have directed them to make changes with the secretary as
needed. I have submitted billing charges in a timely matter and have been working hard on my time
management skills. I need to continue to improve determining and ordering priorities.

understanding of basic sciences and entry level exam/eval/interventions. Furthermore, Paige demonstrates
excellent documentation skills and time management in the clinic. Paige has consistently sought out, accepted

At this time, I am currently capable of managing 60-80% of the caseload with assistance.

Self Final — Paige Papineau A full caseload for an new grad is 7-9 patients per day. I am currently capable of managing 90-100% of the
caseload with minimal assistance.
