We were advised by the technical department but could not arrive at a definite cause.
As a conclusion, we propose to replace the C.Main and C.Term of the C-operation unit under warranty,
although it is a prospective parts replacement.
(It seems that M-operation was also frozen, but there is no past case that two units were frozen at the
same time. )
I would wait for your information.

Please find attached the requested data collected as per maker advice.

aa) Both G1/G2 displays in C op OR G1 or G2, C op could not operate? ‒ GMZ900 was completely frozen,
Gyros input could not be selected.
bb) Neither G1/G2 display in the electrical room OR G1 or G2, M op could not operate? ‒ Both Gyros G1 &
G2 display in the electrical room was frozen even when the vessel altered its heading.
cc) Repeater display & PCR record ‒ Repeater displays were frozen.
dd) Azimuth display of radar & ECDIS, etc. ‒ Azimuth displays of radar and ECDIS were reported frozen.

Weather during the time of incident: Rough Seas and Moderate swell. No Thunderstorm or lightening was reported.

In this case, we think Presumed factors are: C-op or M-op or changeover unit failure, crew mishandling,
lightning, etc.)
Therefore, we would like to check the command history and error history of Gyro as material for
understanding the current situation.
We would appreciate it if you could fill in the command history and error history in the attached excel file.
Also, just to be sure, please reconfirm which of the following was the cause of the orientation freeze.
aa)Both G1/G2 displays in C op OR G1 or G2, C op could not operate?
bb)Neither G1/G2 display in the electrical room OR G1 or G2, M op could not operate?

Subject: STARWAY‐ GYRO SYNCRONIZATION LOST FOR SHORT TIME

Good day,

Please note vessel has experience Gyro synchronization lost for short time on 17 dec 2022/1910 hrs during sailing to
Hound point. Vessel has started hand steering using Magnetic compass and corrected the course. The gyro indication
monitored and since the incident occurred and observed in order without any error or malfunction.

Course of events:
17 dec 2022
1911 hrs‐ Bridge team observed that vessel is altering to Stbd. Gyro heading is frozen and ROT is zero. On gyrocompass
operation unit GMZ900 gyro heading for both units is frozen.
1912 hrs‐ Master called on bridge and Vessel on hand steering, course corrected using Magnetic compass. Tried to
change over to TMC but panel is frozen. During this time several alarms received that Speed is not updated (speed was
on auto).
Also received an alarm that Synchronization lost. Tried to make photo of alarm but disappear from display. Checked
both Master gyro from Electrical equipment room and found that while vessel is altering the gyro heading is frozen.

1915 hrs Vessel on TMC and all repeaters synchronized.
Vessel is correcting the course.
1922 hrs Changed back to Gyro heading input and found in good working condition.

During next hours received a few times alarm that Speed input lost but alarm disappear after acknowledge.

Training carried out with Bridge team for close monitoring of gyro heading while on auto pilot along with procedure for
changing over to TMC/ hand steering.
