Skills Checklist: ADMINISTERING AN INTRAMUSCULAR INJECTION
Learning Objectives: Students will be able to administer drugs correctly via the intramuscular route.

Purpose: To provide a medication the client requires (see specific drug action).

Equipment:
• Client’s MAR
• Sterile medication (usually provided in an ampule or vial or prefilled syringe)
• Syringe and needle of a size appropriate for the amount and type of solution to be administered
• Antiseptic swabs / wet and dry cotton balls, micropore tape
• Clean gloves (according to agency protocol)

ASSESSMENT

Allergies to medication.

Specific drug action, side effects, and adverse reactions.

Tissue integrity of the selected site.

Client’s age and weight to determine site and needle size.

Client’s ability or willingness to participate.

IMPLEMENTATION

1. Check the MAR.

2. Organize the equipment.

3. Perform hand hygiene.

4. Prepare the medication from the vial or ampule for drug withdrawal.

5. Introduce yourself.

6. Verify the client’s identity.

7. Explain the procedure and purpose of the medication.

8. Provide client privacy.

9. Prepare the client.

10. Select and locate the most appropriate site: deltoid area.

11. Apply clean gloves.


12. Locate the upper landmark for the deltoid site by placing four fingers across the
deltoid muscle.
13. Cleanse the skin at the site using firm, circular motion starting at the center
and widening the circle outward.
14. Allow skin to dry.
15. Place a dry cotton ball between the third and fourth fingers of the non-
dominant hand.
16. Remove the needle cover and expel any air bubbles from the syringe.
17. Using the non-dominant hand, firmly pressing the deltoid muscle for 10
seconds.
18. Hold the syringe between the thumb and forefinger (as if holding a pen) of the
dominant hand.
19. Pierce the skin quickly and smoothly at a 900 angle into the muscle.

20. Hold the barrel of the syringe steady with your non-dominant hand.
21. Aspirate by pulling back on the plunger with your dominant hand for 5-10
22. If blood does not appear, inject the medication steadily and slowly.

23. Withdraw the needle smoothly at the same angle of insertion.


24. Release the skin. Apply gentle pressure at the site with a dry cotton ball.

25. Proper disposal of the materials used.


26. Remove and discard gloves.

27. Perform hand hygiene.


EVALUATION

Conduct appropriate follow-up such as desired effect (e.g., relief of pain, lowered body
temperature) any adverse effects (e.g., nausea, vomiting, and skin rash). Report
deviations from normal to the primary care provider. Complete documentation.
