PURPOSE
To establish guidelines for deployment of safe and effective confined space rescue
operations.

SCOPE
This procedure applies to all confined space rescue activities conducted at AMAALA
International Airport Project. The project’s Confined Space Rescue Team will provide
primary emergency rescue support for all permit required confined space entries on the
project (Box Culvert, Diesel Tank, Underground Water Tanks, Mechanical/Electrical
Manholes).

RESPONSIBILITIES
3.1 Managers
• Ensure requirements for entry have been completed before entry is authorized.
• Provide resources to conduct confined space rescue.
• Assure rescue team members are properly trained and equipped.
• Ensure that the rescue team regularly simulate rescue in a confined space. Know the
hazards that may be faced during entry, signs or symptoms, and consequences of
exposure.
3.2 Entry Supervisors
• Determining if conditions are acceptable for entry.
• Authorizing entry and overseeing entry operations.
• Terminating entry procedures as required.
• Ensuring measures are in place to keep unauthorized personnel clear of the area.
• Ensuring that necessary information on chemical hazards is kept at the worksite for
the employees or rescue team.
• Ensuring a rescue team is available and instructed in their rescue duties (i.e., an
onsite team or a prearranged outside rescue service).

3.2 Topman / Entry Attendants


• Be knowledgeable of and be able to recognize potential confined space hazards.
• Maintain a sign-in/sign-out log with a count of all persons in the confined space, and
ensure all entrants sign in and out.
• Monitor surrounding activities to ensure the safety of personnel.
• Maintain effective and continuous communication with personnel during confined
space entry, work, and exit.
• Order personnel to evacuate the confined space if he:
a. observes a condition which is not allowed on the entry permit.
b. notices the entrants acting strangely, possibly as a result of exposure to
hazardous substances.
c. notices a situation outside the confined space which could endanger
personnel.
d. notices a hazard within the confined space that has not been previously
recognized or taken into consideration.
3.3 Entrants
Employees who are granted permission to enter a confined space shall:

• Read and observe the entry permit requirements.


• Remain alert to the hazards that could be encountered while in the confined space.
• Properly use the personal protective equipment that is required by the permit.
• Immediately exit the confined space when:
a. they are ordered to do so by an authorized person.
b. they notice or recognize signs or symptoms of exposure.
c. a prohibited condition exists.
• Alert Attendant(s) when a prohibited condition exists and/or when warning signs or
symptoms of exposure exist.

3.4 Rescue Team Lead


Upon arriving on the scene, Rescue Team Lead will assess the following and advise all other
personnel responding:

• What type of space is this?


• Are there product storage hazards?
• Locate and secure the attendant or a reliable witness.
• Determine location and number of victims.
• Obtain blueprints, maps or have site personnel draw a sketch of the site.
• Determine the mechanisms of entrapment or nature of illness.
• Make a conscious decision as to this is a "rescue or recovery".
• Determine number of entry points and locations.
• Determine electrical/mechanical/chemical hazards.
• Directs the entire emergency on scene.
• Is responsible for the tactical activities and decisions.
3.5 Rescue Team Members
• Communicates with the Rescue Team Lead.
• Performs actions as directed by the Rescue Team Lead
3.6 Security
• Assure all security personnel are trained in their roles in the rescue process.
• Maintain site security at the confined space rescue scene.
3.7 Site Nurse
• Respond to any site emergency call in a timely manner.
• Provide first aid treatment to those in need of urgent medical care on incident scene.
• Determine nature and extent of illness or injury, establish first aid procedures to be
followed or need for additional assistance.
• Ensure all confined space rescue team personnel receive appropriate medical
examinations.

• Cares for and transports sick or injured patients to medical facility, working as
member of emergency medical team.
3.8 Ambulance Driver
• Responds to instructions from medical emergency dispatcher and drives the
ambulance to specified location.
• Assist site nurse to transport sick or injured patients to medical facility, working as
member of emergency medical team.

RESCUE PROCEDURE
4.1 Notification
• Upon identifying an emergency, the attendant shall immediately notify the site nurse
and the confined space rescue team via the site emergency contact number
(0555016400)
• Upon notification of a confined space rescue situation, the following will be
dispatched to the incident location.
i. Site nurse and Ambulance
ii. Confined space rescue equipment and all emergency rescue personnel

4.2 Phase 1 Arrive On-Scene. Take Command. Size-Up.

4.21 The Primary Assessment


• Secure a responsible party or witness to the accident, determine exactly what
happened and how many victims have been affected.
• An immediate assessment of the hazards present to rescuers must be done.
• An early decision must be made as to whether the operation will be run in rescue or
recovery mode.
• Establish communications with the victim as soon as possible.
• Locate confined space permit and all other information about the space.
4.22 The Secondary Assessment
• The Confined Space
i. Emergency rescue team lead to determine what type of confined space this
is. This can be done by consulting the type of entry permit.
ii. What known hazards are present; mechanical, electrical, etc.
iii. Location and number of victims affected.
iv. Diagram of confined space, including entry and egress locations.
v. Structural stability of the confined space.
vi. Hazardous material size-up.
• On-Scene Personnel and Equipment
i. Determine if there is an adequate number of properly trained personnel on
scene to do the rescue/recovery.
ii. Consider the effect of extreme temperature on personnel and consider early
rotation of personnel operating on scene.
iii. Consider if the proper equipment is on-scene to complete the operation. This
includes, but is not limited to:
- Atmospheric monitoring equipment
- Intrinsically safe lighting.
- Supplied air breathing apparatus or remote air.

- Victim retrieval systems/equipment e.g., rescue tripod, wheel


stretcher.
- Ventilation equipment.
- Ambulance.
4.3 Phase II Pre-Rescue Operations

4.31 Make the General Area Safe


• Establish a perimeter. The size of the perimeter should be dictated by the
atmospheric conditions, wind direction, structural stability, etc.
• Stop all unnecessary traffic in the area.
• Ensure that vehicles park downwind from incident if vehicles are running.

4.32 Make the Rescue Area Safe


• Determine exactly what hazards and products are within the confined space.
• Carry out atmospheric testing in the space to determine oxygen level, flammability,
and toxicity. Based on readings, proper level of personal protective equipment will
be determined.
• Any product that is in or flowing in the confined space must be secured if possible.
• Any running equipment must be shut down prior to entry if it is a run-in rescue. If
possible, all equipment should be locked/tagged out and brought to a zero-energy
state.
• The structural stability of the confined space should be evaluated. If there is a
potential for collapse, appropriate measures must be taken to ensure the structural
stability of the space.
• Ensure proper ventilation of the confined space.
• Also consider the effects the exhaust is having on the operation. Don't add CO to a
confined space.

4.4 Phase III Entry Operations/Victim Removal.

4.41 Selection of Personnel


• If entry into the confined space is required to rescue victim, proper personnel shall
be selected to make entry. A minimum of two persons should be assigned to make
entry.
• All entry and back-up personnel should be properly trained in confined space rescue
procedures.
4.42 Communication and Lighting
• A proper communication system must be established. This will be accomplished
using either rope pull, whistles or blow horn as applicable.
• If the entry/rescue team is entering a dark confined space proper type of lighting
must be used. If explosion proof lighting is not available, then calumet type lights
must be used by the entry team.
4.43 Orientation of Confined Space
Confined space on the project can be generally classified into two based on their entry
orientation.

a) Vertical entry: Confined spaces with narrow opening on the top such as mechanical
manholes, Electrical manholes, Underground water tanks, Diesel tanks.
b) Horizontal entry: Wide Box culverts, GRP pipes

4.44 Assessing Condition of Victim

• Upon reaching the victim, entry personnel should do an immediate primary


survey of the victim. If appropriate, treatment should be started immediately.
• A quick but thorough secondary assessment of the victim should be done. If
time permits, entry personnel should attempt to treat serious injuries prior to
removal.

4.45 Victim Removal Equipment/ System


Prior to removal of victim, the rescue team should have determined the appropriate
method/ equipment of victim retrieval depending on the confined space entry orientation.

a) For vertical rescue: For manholes emergency rescue tripod will be used to retrieve
the victim. Before starting work in the confined space, all entrants must wear a body
harness and attach its “D” ring to the tripod's lifeline. Rescuer can then easily
retrieve the victim in the event of an emergency.

b) For Horizontal rescue: In the case of pipe or box culvert rescue, a stretcher with
wheels will be used. Rescue team will go inside through the wide entrance with the
rescue equipment, position the victim on top of the stretcher and transport him out.
NOTE: IF the victim is a fatality, you may want to leave the body and related
equipment in place for investigative purposes.

4.46 Transfer to Treatment


• Immediately after reaching the point of egress, entry/rescue personnel shall transfer
the victim to treatment personnel (site nurse)

TRAINING & EMERGENCY RESPONSE DRILL:


5.1 Training
Employees engaged in work in confined spaces is required to be properly trained, and it is
the employer's responsibility to provide adequate training, information, and supervision to
ensure, as far as is reasonably practical, that the employees' health and safety is protected.

Training must be planned specifically for:

a. Supervisors
b. Persons who will be expected to enter and work in confined space.
c. Persons who will act as attendants.
d. Persons appointed to form a rescue team.
The precise form of the training and instruction must depend on the individual operations,
but in addition to any specialized training for tasks, general training for work in confined
spaces should include:

i. Observance of the safe system of work (permit-to work)


ii. Instruction in the use of atmosphere testing equipment, suitable breathing
apparatus, and practice in their use and maintenance
iii. Training in the procedures for rescue, including the correct use and maintenance of
rescue and resuscitation equipment.
iv. Instruction and practice in the correct procedures in emergencies, especially
evacuation and use of firefighting equipment
v. (Where applicable) observance of personal hygiene rules to avoid health risks.
vi. Practice drills are an essential part of training theoretical knowledge is not enough to
ensure that the right action will be taken in a real emergency.
vii. It is important that a full and up to date record should be kept of the type of training
given to everyone.

5.2 Emergency Response Drill


Emergency response drill, coordinated by the HSE Department will be conducted on site
periodically, to determine the readiness and effectiveness of existing plans. Any
observations or suggestions for improvements for better response will be recorded and
incorporated.

Emergence evacuation drill should ensure that employees acquire a sound working
knowledge of the signal communicating system to be used between persons working in the
confined space and attendant. They must also learn the correct procedure for summoning
medical aid or the emergency services, and the use and maintenance of any recovery
winches and/or other methods of recovery.

Emergency response equipment shall also be inspected at routine intervals to ensure that
they are always fully functional, and where required calibrated as per manufacturer's
specifications.
