SL Neqas (BGS) : REPORT FORM (March 2023)

This document contains a report form for Sri Lanka's National External Quality Assessment Scheme for Blood Group Serology. It provides results from testing samples for ABO and RhD blood grouping, as well as antibody screening and crossmatching. For one sample, the direct antiglobulin test was positive, indicating an immune cause such as autoimmune hemolytic anemia. Two samples showed unexpected antibodies on screening which would require identification before non-emergency transfusion. One crossmatch was incompatible, suggesting the presence of clinically significant antibodies.

Q1- Would you refer any of the above samples to the Reference IH laboratory before a non-
emergency transfusion? If yes, for what reason?
No

Patient 2 0 Positive ☐ Negative ☒
Patient 3 1+ Positive ☒ Negative ☐

Q2- Would you refer any of the above samples to the Reference IH laboratory? If yes, for what
reason? What are the possibilities for your findings related to this sample?
No. Inform CTP. Do the DAT profile in the Blood bank.
autoimmune hemolytic anemia, hemolytic disease of the newborn, transfusion reactions

9. Screening for unexpected antibodies (Please grade and record reaction results)
(You may use 2 cell or 3 cell panel, as available for your routine practice)

Q3- Would you refer any of the above samples to the Reference IH laboratory before a non-
emergency transfusion? If yes, for what reason? If in an emergency, how would you issue blood
for this patient?
Yes.
For antibody identification
In an emergency, issue uncrossmatched O negative blood and if time permits issue Group
specific IAT cross matched blood (LISS method)

10. Cross match (Please grade and record reaction results)

 Perform all 6 cross matches disregarding the ABO/Rh D groups of the patient/donor
samples
Patient 1

Compatible ☒
Donor Y 0 0 0 select. Incompatible ☐
Compatible ☒
Donor Z 0 0 0 select. Incompatible ☐

Patient 3

Compatible ☐
Donor Y 3+ 2+ 3+ select. Incompatible ☒
Compatible ☒
Donor Z 0 0 0 select. Incompatible ☐
(RT = Room Temperature)
Q4- If any of the above shows incompatibility what could be the reason for it?
Donor Y- Blood group is B positive | Donor Z- Blood group is O negative.
P1/Donor Y- P1 is AB positive and Donor Y is B positive. So it can be a minor blood group
incompatibility ( antibodies in P1 sample clumping with donor cells)
P3/Donor Y- P3 is O negative and Donor Y is B positive, so it is a ABO blood group
incompatibility.
Please perform testing as early as possible
(Preferably within 3 days of receiving samples)
