Announcement (READ ME FIRST): Good day future nurses aka healthcare frontliners. Welcome to our Class in
Community Health Nursing-1 (Theory). This course deals with concepts, principles, theories and techniques in the
provision of basic care in terms of health promotion, disease prevention, restoration and maintenance and
rehabilitation at the individual and family level. It includes the study of the Philippine Health care Delivery System,
national health situation and global context of public health. The learners are expected to provide safe, appropriate
and holistic nursing care to individual and family as clients in community setting utilizing the nursing process.
This is 4 units course, 2 units in theory and 2 units in RLE. We are expected to complete 36 hours of didactic and 102
hours in clinical duty (Skills Lab=51 hrs & Clinical=51 hrs). We are going to have at least one hour per week every
Monday between 9AM to 10AM online classes through Zoom. This might be complex and challenging journey knowing
that this is our new norm in teaching-learning process due to pandemics. However, we must be enthusiastic and get
motivated to embrace and adapt to this paradigm shift in education. Let us always be optimistic and always look in
the silver lining of this experience. We can do it together.
So, let’s us start the journey. Always remember that We/I learn as one that is why CPAC-SON designed “I CARED”
model for you. This stands for:

Please read each module carefully. Accomplish your learning activities and assignments according to the time frame
given. Take note, the module will collapse after the due date and you cannot go over again. Manage your time wisely
and if you have any questions or clarifications, feel free to communicate with me in our GC or my Facebook
messenger. As you start each module, always start by clicking the Learning Design first so you will be guided

MODULE 01:
MODULE TITLE: Fundamental Concepts of Community Health Nursing
INITIATE.
Good day Future Frontliners! Welcome to the new normal way of learning!
Warm greetings to everyone! How are you doing during this pandemic? I wish all the best of everything despite the
challenges that we are facing nowadays. The devastating effects of COVID-19 virus are more apparent not only in the
facet of human health (physical & mental) but also social, and even economic downfall. Amidst this crisis, learning
must go on considering this new normal of learning environment. Hope this partnership of teaching- learning process
utilizing new platform or mode of instruction for learning will be an enjoyable experience for all of us. Let’s take a big
leap of faith in this undertaking. Together, we can make it.

AFTER EFFECT.
As we begin our class, I encourage you to be responsible of your own learning. You can do it. I am always here to
assist you. So, let's take a look at the road map this week for us to be guided on what to do in order to attain the target
goals.
Please read the following learning competencies:
LEARNING OUTCOME: The Level-2 nursing students will apply the fundamental concepts of community health nursing
to respond to family member’s health needs.
Evidence of Learning: Create Word Cloud of fundamental concepts of CHN with at least 5-8 sentences interpretation.
Before we can accomplish this, we need first to look into:
ENABLING OUTCOMES: By the end of the module 1, the learners will have:
1. Discussed the definition of health, community & public health nursing, as well the principles & Standards of CHN
Assessment: Interview You
2. Relate roles (public health interventions) of public health nurses and the Intervention Wheel in their own
community
Assessment: Ask at least 2 neighbors or consumers of health care about their views of health services they received in
their community.
3. Analyzed the determinants of health and diseases in their community.
Assessment: Assessment of their own community (based on determinants of health and diseases as observed in their
own community). Critical Thinking: Thinking Upstream (Examining the root causes of poor health)
IFL OUTCOME: Student nurses should always ask the guidance of the Holy Spirit as they take care of their patients.

REMEDY.
Now that you are familiar with the after effect or outcomes for this module, we will now take the remedy or
treatment plan for you to be able to achieve these outcomes. Let us begin with journey reading to ensure that you
have the full knowledge of the concepts. This module covers Chapter 1 of your textbook in Community Health Nursing
(green book) by Famorca, Nies & McEwen as your reference. Please read this chapter in advance so we can have a
lively and interactive study.
Digestible-Bites Teaching Strategies – We will have 1-hour lecture discussion via zoom at 9:00 AM every Monday. You
can also read articles and other supplemental readings found in the Resources Section in preparation for our
discussion.
Let's start this module by covering first the fundamental concepts in community health nursing. Basically, we have
to define health, community, public health, community health & public health nursing. These principles & standards

of CHN, determinants and indicators of health and disease, roles & responsibilities of public health nurse must be
clearly understood. These concepts are simple and common yet need a deeper understanding. Let’s explore and
tackle the following lessons.
After your journey reading on Chapter 1 of your textbook, answer the following thinking questions posted in Forums
and post your responses by attaching word document of your answers in that section.
2. Define community considering (type & attributes according to Maurer & Smith) the locality or community
where you reside now.
3. Integrate the fundamental concepts of CHN (philosophies & principles) in promoting level the health of
people in your community during this worldwide health crisis.
Forum IE. Critical Thinking: Thinking Upstream (Examining the root causes of poor health) (August 28)
Instruction: Write an essay in 2-3 paragraphs (at least 3 sentences for each paragraph). Examine the root causes of
poor health in your own locality. Identify other determinants of health and diseases. What are the roles the
community health nurses must play to promote health of the individuals, families & community levels.
How are you doing so far? I hope you have already the grasp of the basic concepts regarding the topic and will be
ready put your ideas organized. However, in this phase I believe there are still points or ideas that you cannot fully
comprehend. To address this, you will do the following activity.
FORUM 1F: MUDDIEST POINT, LET ME SEE CLEARLY! (due August 29)
Instructions: Choose a partner from your classmates. After listening to the lecture and in your journey reading, take
note of the “muddiest point”, those concepts that you have a hard time understanding or the most confusing points in
your reading. Ask your partner to discuss the matter with you and help you understand that concept better. Submit a
“muddiest point” report detailing the explanation on the point you have discussed with your partner. Post your
report paper in Forum 1F. Comment on how your partner helped you understood the concept or idea better.
Instructions: Now that you have fully understood the concepts, you are ready to organize your ideas and concepts. At
this juncture, you will learn how to make a Word Cloud. A Word Cloud (also known as text clouds or tag clouds) is a
collection, or cluster, of words depicted in different sizes. The bigger and bolder the word appears, the more often it’s
mentioned within a given text and the more important it is. It is a visual organization of words or text that represents

You create a Word Cloud by writing key words. After understanding how to make word cloud, now it’s your turn to
make one considering all the fundamental concepts in CHN that you learned from this module.
Dyads in evaluating created Word Cloud about the fundamental concepts of CHN.
Instruction- Before you submit the output CHN Word Cloud, share it to your partner and your partner will post his/
her comments in the forum regarding your output. Person A will post Person B's work, and Person B will share
Person A's work. Both of your work should be posted in a STRING/nested manner. However, you are required to
comment on your friend's output. Guide questions when reflecting on your pair's work are:
a. What do you I like about my partner's personalized word cloud?

b. What unique features does this structure have that attract me most?
Also, commends your partner both for the strength & weaknesses. Please give your honest and fair evaluation. You
can do it.
EMPHASIZE
Forum 1I. This is It! CHN Word Cloud (due August 31)
Instruction: Post the final created Word Cloud in the Forum 1I. Now, that we are done with all the learning tasks,
emphasize in this section the important learning points that you have grasp in this learning unit. Write down your
explanation, interpretation or reflection in 3-5 paragraphs (at least 3-5 sentences per paragraph). First paragraph, is
the introduction, 2nd - 4th paragraphs as the body and 5th paragraph as your conclusion. Post this in Forum 1I section.
