1. What behaviours or lifestyle habits do you currently engage in regularly that you believe
support your health? Explain.

are self-destructive? Explain.
3. Who do you know in your life that will sincerely and consistently support you with the
beneficial lifestyle changes that you will be making? How will they support you? What
can you ask them to do to support you?
4. What is your present level of stress (psychological, physical, workplace etc.)? Identify
how stressed you feel on a level from 0 = being not stressed at all, to 10 = being
completely overwhelmed and consumed with stress.
5. Identify 1 of the biggest stressors in your life right now (if not right now, think about
within the last year). For the stressor:
Identify what it is.
State which category/type of stressor it is and explain why.
Provide an example of a positive coping strategy that you can use to help you
deal with this stress.
Explain how this stress has affected you. What parts of your life (or dimensions
of your wellness) has it impacted? Explain.
6. Which dimension of wellness do you believe needs the most improvement in your life
right now? Why do you think this is?
7. What can you do in your daily life to ensure that this dimension of wellness receives
more attention? What can you change or do differently?
8. Which dimension of wellness is the fullest in your life currently? Explain why this, and
what actions you take on a regular basis that contribute to this dimension.
9. a) What goals would you like to achieve this year in terms of your own personal health
and wellness? Ask yourself – what do you want more of in your life? Explain 1 or 2
personal goals that you will set for yourself and how you will achieve them.
b) Identify which stages of healthy behaviour change you are in for these goals. Explain.
