AFFIDAVIT OF LOSS

I, NICOLE A. PORSUELO, Filipino, of legal age, and a resident of Brgy. Zone 4, Talisay City,
Negros Occidental, Philippines, after being duly sworn in accordance with law, depose and state that:

1. I am an owner of a blue Vivo Y11 cellphone with Globe Sim Card with number _________;

2. That the said cellphone was installed a gcash application with gcash credits.

3. That on December 18, 2022, while inside a thrift store, I discovered that such cellphone was
missing;

4. That from that incident until the execution of this Affidavit, I have exerted all means and efforts to
locate said cell phone with Globe sim card but to no avail;

5. Despite my diligent efforts to locate the above-described sim card with number
_________________ for more than weeks now, the same however proved to be futile and to
date, I am unable to locate the said sim card, thus, it may now be declared lost;

6. Thus, I execute this affidavit to attest to the veracity of the foregoing facts and for whatever legal
purpose that this Affidavit may serve.

AFFIANT FURTHER SAYETH NAUGHT.
