CHIEF COMPLAINTS
According to the Informant:
• Not interacting with anyone and not even coming out of his room
• Uncontrolled anger
• Sleep is reduced and taking very less food that too daily once

HISTORY OF PRESENT ILLNESS (HOPI)

Vipin is 21-year B.Tech 3rd year student losses his father 2.5 years back due to covid-19 and
not able to come out of that situation and failed to attempt exams and failed for two years and
used to consume alcohol to forget the grief and added smoking to it as part of habit.

He’s not even coming out of his room for almost 2 years and not interacting with anyone
except his mother

No history of any mental illness

FAMILY HISTORY
• Loss his father due to covid-19 and he loves his father most.
• No psychiatric illness in the family.
• Nuclear family with one younger brother & widowed mother.

Purpose: To assess current behavior and mental state of client and to build rapport.

T: Hi Vipin, good evening!


C: Good evening, sir
T: Hope you’re comfortable here
C: Fine
T: How was your day and what’s your daily routine?
C: I’m third year CSC student, usually go to college everyday but now-a-days not really
interested to go or to meet any one
T: May I know the reason
C: I really don’t know, after my dad’s death I’m not able to face any one or not really
interested to meet anyone all I wanted is be alone.
T: Whom you love & care most in your family from childhood?
C: My father, he’s my hero
T: Other than your father?
C: My mother and brother both are really lovable and they cares for me a lot, but I’m
suffering them with my behaviour may be.
T: Have you ever think about your mother’s situation of losing her husband and worrying
about you
C: That’s really killing me, and I’m not able to forget any memory of my father and I
can’t live without him
T: Ok let’s do some breathing exercise
C: Is it really helpful
T: Yes, these kind of techniques helps to overcome grief

Purpose: To introduce basic techniques to overcome grief and improving behavioural


patterns.

T: Hi Vipin, good afternoon!


C: Good Afternoon, sir
T: How you felt these two days, any change?
C: No sir, but I have few doubts and want to share few things personally
T: Please go on, and you can share anything here with me.
C: Actually I have asked my dad to take me out in lockdown for some useless thing, he
denied many times concerning the outside situation of covid, but I have forced my dad
and that triggers covid to him and leads to death
I’m the reason behind my dad’s death sir, i’m the only reason. (Started crying)
T: It’s okay Vipin, I can understand your pain. situations are like that, don’t feel guilt of what
happens. Whatever happens is done, please focus on what should be done in future
C: I don’t have any future sir
T: Think about your mother & brother, how they’re worrying about your future
C: That’s killing me sir, I’m a big pain in the family now
T: Your never pain for them, at least think about your father, what he thought of you and
dreamed about you.
C: I’m his best friend sir, we used to have lots of time together. All that memories are
making me hopeless.
T: Make those memories your strength, feel your father with you and full fill his dreams and
take care of your family.
C: Sure sir, will definitely try to do
T: First quit alcohol and go to college
C: Yes, sir definitely and also will practise the breathing exercise too.

DIAGNOSTIC FORMULATION

Vipin is 21-year B.Tech 2nd year student approached counsellor along with his elder brother to
overcome the grief due to Loss of his father 2.5 years back due to covid-19 and he’s not able
to cope up the situation and not interacting with anyone and started being alone and
consuming alcohol and avoids college and accumulated backlogs.

Provisional Diagnosis
The client is diagnosed with Prolonged Grief Disorder. Which affecting his social life and
causing emotional pain, e.g. sadness, guilt, anger, denial, blame, difficulty accepting the
death.
Code: Prolonged Grief Disorder (6B42 as per ICD 11)

Points in favour:
• Not able to meet any one
• Disturbed sleeps
• Always being sad and feeling emotional pain

PLAN OF ACTION

Recommendation
After this session, the patient's general feedback related to his feelings and how he was feeling
overall and after mindfulness meditation exercise was recorded.

Patient was further advised and recommended to a senior Clinical Psychologist for the
insights and to carry out further psychological intervention related to her case. Recommended
therapies as per now are Cognitive behavioural therapy, Grief therapy, mindfulness and
positive psychology.
