IV fluids via saline lock.
Perform hand hygiene.
Gather supplies – IV
fluids with primed
primary tubing, IV start
kit, primed saline lock,
lidocaine syringe,
alcohol pad
Introduce self to client.
Ensure privacy.
Identify client using
two identifiers.
Assess extremities for
appropriate placement
for IV insertion.
Apply clean gloves.
Recheck to make sure
all equipment is
accessible.
Apply tourniquet 4-6
inches above proposed
insertion site.
Prepare insertion site
with chlorhexidine
(CHG). Allow to dry
completely – 3 minutes
Anchor skin. Inject
prepared 0.1 mL
lidocaine into
intradermal tissue
lateral & distal to the
intended IV insertion
site.
Again, anchor skin
below the intended
insertion site.
Perform venipuncture
while holding
angiocath at 10°-30°
angle with bevel up.
Look for blood return
in flashback chamber.
Gently advance
catheter off stylet (hub
to skin).
Release tourniquet.
Apply pressure above
the catheter tip with
opposing thumb.
Engage safety device to
withdraw stylet from
catheter.
Aseptically attach
flushed saline lock.
Apply opsite over
insertion site; below
catheter hub &
connection.
Aseptically attach
primed tubing to the
saline lock.
Tape tubing securely to
arm.
Verbalize prescribed IV
rate.
Dispose needles in
sharps container.
Remove gloves.
Provide for client
safety.
Documentation.

Verify provider
prescription for IV fluid
discontinuation &
administration of IV
push medication.
Gather appropriate
supplies – IV push
medication, one saline
flush, alcohol pad
Introduce self to client.
Provide privacy.
Identify client using
two identifiers.
Apply clean gloves.
Verbalize closing of the
IV tubing roller clamp.
Assess IV site.
Remove the tape that
is securing the tubing.
Remove tubing from
saline lock.
Aseptically attach IV
push medication to
saline lock. Push IV
medication as
indicated. Remove IV
push medication
syringe.
Aseptically attach
prepared saline flush.
Over 10-15 seconds
flush saline lock.
Remove saline flush.
Dispose of IV fluids &
tubing. Remove gloves.
Provide for client
safety.
Documentation.

Verify provider
prescription for
discontinuation of IV
site and IV
medications.
Gather appropriate
supplies – 2x2 gauze
pad, tape
Introduce self to client.
Identify client using
two identifiers.
Apply clean gloves.
Carefully remove the IV
site dressing.
Apply a 2x2 gauze pad
above the IV insertion
site & gently remove
the catheter.
Hold pressure to
insertion site; assess
for bleeding, adhere
dressing as appropriate
Inspect catheter tip to
ensure intactness.
Dispose of saline
lock/supplies as
appropriate
Remove gloves.
Provide for client
safety.
