FOR H-1B PETITION
Instructions:
1. Please submit 2 sets of legible photocopies of the following unaltered original documents
2. .Reduce any large documents to 8½” x 11" standard letter size paper.
3. Be sure that all documents copy clearly and legibly, including red ink entry stamps found on
passport and I-94 card.
4. Staple multiple page documents together (example, passport pages)

VERIFICATION OF ACCURACY OF INFORMATION PROVIDED
I hereby certify that all documents submitted in conjunction with my H-1B petition are true copies of unaltered
originals. In addition, all of the information provided to the law firm in my questionnaire is accurate to the best of
my knowledge and belief. I agree to keep the law firm advised of any change of circumstances, travel plans, change
of address or change of employment. If I am currently in H-1B status I understand that I may at my option take
advantage of H-1B portability provisions enacted under the 2000 H-1B legislation and begin work upon filing. I
also understand that there may be questions or issues with my decision to start work under portability prior to
approval, and that I should seek out an individual consultation with the law office prior to making a decision. If I
am outside the USA I understand that I should make a copy of my entry document form I-94 and my H-1B visa
stamp and mail a copy to the law firm upon entry into the USA in H-1B status. I understand that this checklist is the
