3.5 Phenol

The document discusses various intracanal medicaments used in endodontic treatment, including their compositions, uses, advantages, and disadvantages. It provides details on parachlorophenol (PCP) and camphorated monoparachlorophenol (CMCP), which are commonly used. PCP is the active ingredient and camphor is added to prolong the antimicrobial effect and reduce irritation. The document also presents two case examples where intracanal medicaments were used - phenol in one case for a mandibular molar with periapical lesions, and in another case for a canine with apical periodontitis. References are provided.

1. Composition

Parachlorophenol
● It is substitution product of phenol in which chlorine replaces one of the hydrogen
atoms (C6H4OHCl)
● On trituration with gum camphor, these products com- bine to form an oily liquid

Camphorated Monoparachlorophenol (CMCP)

Two parts of PCP

+
Three parts gum camphor

Camphorated monochlorophenol (CMCP)

Cresatin
● It is a clear, stable, oily liquid of low volatile nature known as metacresyl acetate

2. Uses

Phenol
It was used for many years for its disinfectant and caustic action. However, it has strong
inflammatory potential, so, at present, it is rarely used as an intracanal medicament.

Uses
1. Disinfection before periapical surgery
2. Cauterize tissue tags that resist removal with broaches or files

Parachlorophenol (PCP)
Parachlorophenol (PCP) has been a very popular component of dressing as phenol is no
longer used in endodontics because of its high toxicity-to-efficacy ratio.

Camphorated Monoparachlorophenol (CMCP)

Camphor is added to PCP because it
● Has diluent action
● Prolongs the antimicrobial effect
● Reduces the irritating effect of PCP
● Serves as a vehicle for the solution

Uses
1. Used as dressing of choice for infected tooth

3. Advantages
4. Disadvantages

•Phenol itself is no longer used in endodontics because of its high toxicity, but the
derivative paramonochlorophenol has been a very popular component of
interappointment dressings.
•Antibacterial effect is not long lasting
•They are able to diffuse through the temporary filling material
•Cause an unpleasant taste in the mouth
•Soften the filling material.

5. Example of cases
a) A 43 year female patient reported pain and swelling in left mandibular region for the
past few months. At the time of the first examination, there was spontaneous pain at
the left mandibular first molar. Clinical examination revealed that the tooth had a
deep defective lesion and provisional sealing with temporary restoration at occlusal
aspect. Radiographic analysis showed communication of the temporary restorative
material with the pulp chamber, peri radicular lesions in both roots, and severe
inflammatory apical root resorption in the distal root. After conducting clinical and
radiographic analysis, it was decided that endodontic treatment should be
performed. Initially, the tooth was anaesthetised and cleaning and shaping of canals
were carried out. The canals were irrigated at each change of file, with 2.5 ml of
2.5% sodium hypochlorite, and a final irrigation was performed with EDTA to remove
the smear layer. After that, intracanal medicaments were applied to disinfect the
root canal system in order to reduce the risk of contamination. In this case, phenol
intracanal medicaments were applied prior to obturation procedure and sealing of
the canal to prevent microorganism growth.

b) A 56 years old male patient complained of pain and swelling on the right side of his
lower jaw for a week.clinical examination and radiographrevealed that his manibular
right canine has apical periodontitis.
