PETITION FOR CONFIRMATION OF TALAQ


and ISSUANCE OF DECREE OF DIVORCE

COMES NOW Petitioner, through counsel and unto this Honorable


Court, most respectfully avers that:

1. Petitioner Divorcé RODEL QUIN y FRANCISCO is of legal age,


Filipino citizen, and a resident of Barangay Sto. Niño, Zamboanga
City, while Respondent Divorcée SHADARISNA JAKARIA
TANJILUL is likewise of legal age, Filipino citizen, and a resident of
Barangay Tumaga, Zamboanga City;

2. Herein parties, both with capacity to contract marriage pursuant to


Article `16 of PD 1803, had contracted marriage (nikah) on
September 14, 2011 at the bride’s residence in Tumaga Walk Drive,
Zamboanga City, in accordance with Chapter Two of the said Decree,
solemnized under islamic rites and ceremonies by Imam SUKARNO
R. ALAM, whose judicial affidavit is attached herewith as Annex
“A” and made an integral part hereof;

3. To comply with Paragraph (d), Article 15 of PD 1083, the two (2)


competent witnesses to the said marriage were JULHASBI SAPPARI
and AMILPASIL AMILHUSSIN, whose joint judicial affidavit is
attached herewith as Annex “B” and made an integral part hereof;

4. Cohabitation of herein parties for six (6) years after their marriage
was blessed with _____ (___) children, currently under care and
custody of Respondent, copy of their Certificates of Live Birth are
attached herewith as Annexes “__”, and “__”, and made integral
parts hereof;

5. Sometime in the year 2017, Respondent Shadarisna reverted to her


ways not fit for a Muslimah in terms of her demeanor; attitude
towards Petitioner Rodel; her dress code; and her continuing defiance
to the teachings of Islam, despite the latter’s pleas and constant
reminder;
6. Petitioner had executed an Affidavit pronouncing Talaq on his wife,
copy thereof is attached herewith as Annex “___” and made an
integral part hereof;
