Classroom Observation Form

This teacher observation form is used to evaluate instructors across 10 criteria including subject matter expertise, organization, rapport with students, teaching methods, presentation skills, classroom management, sensitivity, assistance to students, personal attributes, and physical classroom environment. Observers are asked to provide evidence and rate instructors on a scale of 1 to 3 for each standard, with 1 being the lowest and 3 being the highest. Strengths, areas for improvement, and an overall impression of teaching effectiveness are also noted.
