CERTIFICATE OF OCCUPANCY
(ALL FORMS SHOULD BE FILLED-UP)

1. Application Letter; Photocopy of owner’s ID with three (3) specimen signatures;


authorization letter.

2. Zoning Validated Certificate of Occupancy for 1,500 m 2 above

3. Accomplished Certificate of Completion for:


- Building Construction
- Sanitary / Plumbing Installation
- Electrical Installation
- Mechanical Installation

4. Fire Safety Inspection Certificate

5. Picture/s of the subject showing four (4) elevations (3 copies sets each)

6. Photocopy of approved permits;


- Building Construction
- Sanitary / Plumbing Installation
- Electrical Installation
- Mechanical Installation

7. Copy of Approved Plans (1 set)

8. Waiver from previous professional in-charge, if project is undertaken/assigned to others

9. Stuctures with CAAP clearance (submit structural survey for the actual bldg. height)
signed and sealed by Geodetic Engineer
