13 AQ16 CONSOLE ERROR

AQUILION 16-SLICE CT

Defective RTM-D PWB
 The RTM-D PWB can fail in different ways
 It can cause a Task Starter error
 A Transport error
 It can hang up the Scan console boot and never display any error messages as in this case

 The Scan console boot stops at this screen


 No error messages are displayed

 Open the front cover of the CPU box and look at the LED’s on the SS/ADI PWB
 LED1 (CPU-1) should be lit or flashing
 In this case LED1 is not lit

 Remove the left side cover of the Rec. box
 Remove the panel to the right of the RTM PWB to view the LED’s on the RTM-D PWB
 LED D5 is lit
 P-ERR This LED lights when an error occurs during communication with the SS/ADI PWB

 Tip
 If the SS/ADI PWB was causing the problem
LED 12 (PERR) on the SS/ADI PWB would be lit

 You can also see the error in the SS/ADI log

 Make sure you shut down the scanner properly to make sure the software is not corrupted
 On the Scan console
 Press [Ctrl] + [ESC] to bring up the Start menu
 Click Run

 Type sh [Space] –L
 Click OK

 In the shell
 Move to the X partition
 Type x: [Enter]
 Type ScanSysEnd [Enter]

 When ScanSysEnd is complete the following is displayed


 Click Continue

 When the shell returns to the [ X:/ ] prompt
 Type EndFSYS [Enter]

Task Starter Error

 Task Starter error is a common error


 It can be caused by the following
 Defective RTM PWB
 Corrupt RTM boot line
 Defective RTM-D PWB
 Defective MHR PWB
 Defective RDD chassis or RDD disk
 Defective fiber channel loop between the
Scan console and Rec. Box

 To troubleshoot this error


 Remove the left side cover of the Rec. box
 Check the LED DS1 on the RTM PWB (BFL Board Failure). If it is lit yellow, the RTM PWB is defective
 Check the MHR PWB’s
 Do any of the PWB’s have a red LED lit or is there a PWB with no LED’s lit?
 If the answer is “YES”, shutdown the scanner and remove PWB
 Turn the scanner on. If you no longer have the error, the MHR PWB removed is the cause of the error
 Connect your laptop to the COM-1 port on the RTM PWB
 Start Tera Term
 Press the RST (RESET) button on the RTM PWB
 Does the VxWorks logo appear on the laptop screen?
 If the answer is “YES”, are the boot line settings correct?
 If the answer is “NO”, shut the scanner down and remove the 3 MHR-BP PWB’s

 Turn the scanner back on
 Does the VxWorks logo appear on the screen?
 If the answer is “YES”, one of the 3 MHR-BP PWB’s is the cause of the error. Put the PWB’s back in one at a time to
determine which PWB is causing the error
 If the answer is “NO”, shut the scanner down and remove the 3 MHR-DP PWB’s
 Turn the scanner back on
 Does the VxWorks logo appear on the laptop screen?
 If the answer is “YES”, one of the 3 MHR-DP PWB’s is the cause of the error. Put the PWB’s back in one at a
time to determine which PWB is causing the error
 If the answer is “NO”, Shut the scanner down and remove the 2 MHR-FC PWB’s
 Turn the scanner back on
 Does the VxWorks logo appear on the laptop screen?
 If the answer is “YES”, one of the 2 MHR-FC PWB’s is the cause of the error. Put the PWB’s back in one at a
time to determine which PWB is causing the error
 If the VxWorks logo appears and the RTM PWB boots up completely
 Open Windows Explorer and move to X:/ct/KBProj/develop/etc/scan/BIR/bin and double left click FCDView.exe

 A good Fiber channel loop will show 2 Loop Devices (The FC PWB in the Scan console and MHR-FC PWB slot 18)
 A defective Fiber channel loop will show Not Available

Missing MHR-DP or MHR-BP

 Symptoms
 Scanner boots normally
 Warm-up completes normally
 When a scan is attempted
 The gantry rotates but the Start button on the Scan keyboard never lights and the Confirm button in the exam plan lights
 Click the Confirm button, the Abort button on the Scan keyboard will light once, then the Confirm button in the exam plan lights
 You will be stuck in this loop until you quit the exam plan
 After a few minutes the following error message will appear

 Move to the G:/EventLog folder


 Unzip and open the associated BIR log
 Scroll down to Bootup Printfs and Last Printfs Logged on the RTM
 Look closely at the number of G4 processors found. There should be 16 G4 processors
 Look at the board slot numbers to see which board is missing

NO message

 There are 3 things that can cause this message


 Defective MHR-FC PWB slot 19
 Defective GCIFA-D PWB in the gantry
 Defective fiber optic cable between the GCIFA-PWB and the Rec. box

 Very easy to troubleshoot
 Switch the fiber optic cables between MHR-FC PWB slot 18 and MHR-FC PWB slot 19
 If the LED on MHR-FC PWB slot 19 remains red, MHR-FC PWB slot 19 is defective
 If the LED on MHR-FC PWB slot 18 is now red, either the GCIFA-D PWB or the fiber optic cable between the gantry and the Rec.
box is defective. (Most likely the GCIFA-D PWB is defective)

Scan Boot-up hangs up at “Scheduler is now recovering”

 Solution
 Set the Scan console up to boot in “Single-User” mode
 Open a shell window, move to the X: partition and type
 [X:\] chkconfig [Space] aquilion [Space] off [Enter]
 Shut the scanner down
 Power off when it is safe
 Unplug the power cord to the display computer to minimize boot up and shutdown time

 Turn power back on
 The Scan console boots to the System Setup Menu
 Click Create Image Directories
 Click Init Logdata only

 Click Console
 Enter the following in the shell
 [X:/] chkconfig [Space] aquilion [Space] on [Enter]
 Click Quit

Post processing of printout is running

 Symptoms
 This message appears when the scanner is shutting down and hangs up the shut-down process

 Solution
 Open Windows Explorer
 Move to X:/ var/tmp folder
 Delete the file qmgrStart.lock

 The scanner will then continue to shut down
