We are research students under Science, Technology, Engineering, and


Mathematics strand (12-STEM 1) at Pangasinan National High School and
we are now working on a research paper entitled "Awareness on
Comprehensive Sexuality Education of Grade 12 Students: Basis for an
Intervention Program" which fulfills a portion of the criteria of Inquiries,
Investigations, and Immersion.

By this, we would like to request permission to conduct our research study


in our school, as well as permission to allow students to participate as our
respondents by answering our questionnaires. Rest assured that the data
we collect will be kept entirely confidential and used purely for academic
purposes only. We ensure that all COVID-19 safety procedures are followed
and that no harm is done to the respondents.
